摘要
目的观察沙库巴曲缬沙坦(SV)治疗急性失代偿心力衰竭(下称心衰)的疗效。方法选择2018年1月至2020年3月衢州市中医医院收治的急性失代偿心衰患者111例,采用随机数字表法以1∶1.5比例分为观察组46例和对照组65例,均采取常规治疗,观察组在常规治疗基础上应用SV治疗,对照组应用血管紧张素转化酶抑制剂(ACEI)治疗。连续治疗1年后,比较两组患者左心室射血分数(LVEF)、左心室舒张末期内径(LVEDd)、LVEF提升≥5%、LVEF≥50%、LVEDd缩小≥10 mm比例、心血管性再住院、全因死亡和不良反应情况。结果治疗后两组患者LVEF较治疗前升高,且观察组高于对照组,治疗后两组LVEDd较治疗前降低,且观察组低于对照组,差异均有统计学意义(均P<0.05)。观察组患者LVEF提升≥5%、LVEF≥50%、LVEDd缩小≥10 mm比例高于对照组,心衰再住院发生率低于对照组,差异均有统计学意义(均P<0.05)。两组之间全因死亡发生率、不良反应发生率比较,差异均无统计学意义(均P>0.05)。结论急性失代偿心衰患者应用SV有益于改善左心室重构和心功能,减少心衰再入院。
Objective To observe the effect of sacubitril valsartan on the prognosis of patients with acute decompensated heart failure(ADHF). Methods A total of 111 patients admitted with ADHF in Quzhou Hospital of TCM from January 2018 to March 2020 were divided into observation group(n=46) and control group(n=65). Patients in the observation group was treated with sacubitril valsartan and those in the control group was treated with angiotensin converting enzyme inhibitors in addition to conventional treatment. After 1 year of continuous treatment, left ventricular ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDd), rehospitalization rate, all-cause mortality and adverse effects were compared between the two groups. Results LVEF increased and LVEDd decreased after treatment than before in both groups(all P<0.05). LVEF was significantly higher and LVEDd was significantly lower in the observation group than in the control group after treatment(all P<0.05). The proportions of LVEF increased ≥5%, LVEF≥50% and LVEDd decreased ≥10 mm were significantly higher in the observation group than in the control group(all P<0.05). Rehospitalization for heart failure was significantly lower in the observation group than in the control group(P<0.05). All-cause mortality and adverse reaction were similar between the two groups(all P >0.05). Conclusion Sacubitril valsartan is benefit for improving left ventricular remodeling and cardiac function in patients with ADHF and reducing readmission for heart failure.
