海藻生物胶对胃十二指肠穿孔患者穿孔愈合及腹腔感染的影响 被引量:1

Role of seaweed biological glues in perforation healing and abdominal infection in patients with gastroduodenal perforation

摘要 目的 探讨海藻生物胶(SBG)对胃十二指肠穿孔(GDP)患者穿孔愈合(PH)及腹腔感染(AI)的影响。方法 将106例GDP患者随机分为治疗组43例和对照组63例,于术前和术后第3天观察其炎症指标血白细胞(WBC)、C-反应蛋白(CRP)及降钙素原(PCT)水平的变化;同时观察术后穿孔愈合情况、腹腔引流量、肛门排气时间、下床活动时间、伤口愈合情况、有否腹腔脓肿和住院时间,对比两组患者上述指标的变化情况,将所得数据进行统计学分析,探讨海藻生物胶(SBG)在胃十二指肠穿孔(GDP)中应用的安全性和有效性。结果 两组患者的穿孔愈合率均是100%;两组患者术前炎症指标WBC、CRP、PCT对比差异无统计学意义;治疗组术后第3天WBC和CRP对比术前水平有明显下降,而PCT仅稍微好转;对照组WBC和CRP术后第3天水平较术前有所下降但幅度较小,而PCT变化不大;两组炎症指标术后的对比,治疗组WBC和CRP水平对比对照组有明显下降,差异有统计学意义(P <0.05),而PCT的对比虽差异无统计学意义(P=0.64)但治疗组比对照组略有下降。两组患者术后腹腔引流量、排气时间、下床活动时间、切口感染率和腹腔脓肿发生率的对比差异均无统计学意义,但治疗组术后腹腔引流量较对照组有明显下降(P=0.069)。结论 研究发现海藻生物胶应用于胃十二指肠穿孔患者是安全的,不仅能促进穿孔愈合,而且能有效改善腹腔感染。 Objective To explore the role of seaweed biological glues(SBG)on perforation healing(PH) and abdominal infection(AI)in patients with gastroduodenal perforation(GDP).Methods One hundred and six GDP patients were randomly divided into treatment group(n = 43)and control group(n = 63).We detected the levels of white blood cells(WBC),C-reactive protein(CRP)and procalcitonin(PCT)before and 3 days after operation and at the same time we observed the postoperative perforation healing situation,the abdominal drainage,anal exhaust time,ambulation time,wound healing,abdominal abscess,and time of hospital stay.Then we compared the changes of the data between the two groups with statistical analysis,and explored the safety and efficacy of the seaweed biological glues(SBG)in gastroduodenal perforation(GDP).Results The perforation healing rate was 100% in both groups.There was no significant difference in the preoperative inflammatory indicators WBC,CRP and PCT between the two groups.The WBC and CRP levels in the treatment group were significantly decreased on the third day after operation,while PCT was only slightly improved.The levels of WBC and CRP in the control group on the third day after surgery decreased slightly comparing with those before surgery and the level of PCT remained unchanged.After surgery,the levels of WBC and CRP in the treatment group were significantly lower than those in the control group(P<0.05),while the level of PCT was insignificantly decreased in the treatment group comparing with the control group(P = 0.64).There were no differences in postoperative abdominal drainage volume,exhaust time,ambulation time,incision infection rate and incidence of abdominal abscess between the two groups.However,postoperative abdominal drainage volume in the treatment group was marginally significantly lower than that in the control group(P = 0.069).Conclusion Seaweed biological glues are safe in the treatment of gastroduodenal perforation.It can not only promote perforation healing,but also effectively improve abdomi
