自制口鼻通气托架在眼科局部麻醉手术中的应用

摘要 目的:探讨自制具有简易泵吸功能的口鼻通气托架在眼科局部麻醉手术中的应用。方法:选择2019年11—12月在汕头大学·香港中文大学联合汕头国际眼科中心进行眼科局部麻醉下白内障超声乳化摘除联合人工晶体植入术的患者56例。按随机数字法分为观察组(应用自制具有简易泵吸功能的口鼻通气托架)和对照组(采取鼻导管吸氧),每组28例。在手术铺巾前及铺巾后5、10、15 min,监测患者的呼吸频率、血压、脉搏、血氧饱和度和患者呼吸微环境中的氧气浓度、二氧化碳浓度、温度和湿度。术前及术后运用数字模拟评分法对患者的呼吸舒适度进行评分。结果:观察组和对照组呼吸频率、血压、脉搏、血氧饱和度变化均不存在差异(均P>0.05)。观察组在手术铺巾后5、10、15 min时呼吸微环境的温度均低于对照组(P值均<0.05)。术后观察组呼吸困难评分低于对照组[分别为(0.32±0.77)分和(3.37±1.72)分,P<0.05]。结论:具有简易泵吸功能的口鼻通气托架改善了眼科局部麻醉手术患者的呼吸微环境,减轻了呼吸不适感。 Objective:To investigate the application of self-made oronasal ventilation bracket with simple pumping function in ophthalmic local anesthesia surgery.Methods:From November to December 2019,56 patients who underwent phacoemulsification and intraocular lens implantation under local anesthesia in ophthalmology at Joint Shantou International Eye Center of Shantou University and The Chinese University of Hong Kong were selected.According to the random number method,they were divided into observation group(using self-made oronasal ventilation bracket with simple pumping function)and control group(using nasal catheter for oxygen inhalation),28 cases in each group.Before surgical draping and 5,10,and 15 minutes after draping,the patient's respiratory rate,blood pressure,pulse,blood oxygen saturation,and oxygen concentration,carbon dioxide concentration,temperature,and humidity in the patient's respiratory microenvironment were monitored.The breathing comfort of patients was scored by digital analog scoring method before and after operation.Results:There were no differences in respiratory rate,blood pressure,pulse,and blood oxygen saturation between the observation group and the control group(all P>0.05).The temperature of the respiratory microenvironment in the observation group was lower than that in the control group at 5,10,and 15 minutes after surgical draping(all P<0.05).The postoperative dyspnea score in the observation group was lower than that in the control group[(0.32±0.77)points and(3.37±1.72)points respectively,P<0.05].Conclusion:The oronasal ventilation bracket with simple pumping function can improve the respiratory microenvironment of patients undergoing local anesthesia in ophthalmology and relieve respiratory discomfort.
