摘要
目的探讨调脂治疗对老年高血压患者内皮功能及线粒体自噬功能的影响,并研究内皮功能与线粒体自噬功能的相关性。方法选择2015年4月至2017年11月在山东省医学科学院心脑血管病防治研究中心体检的672例患者作为研究对象。根据血压和血脂水平以及用药治疗情况,将患者分为单纯高血压+未调脂组(135例)、高血压合并高脂血症+未调脂组(201例)、单纯高血压+调脂组(163例)及高血压合并高脂血症+调脂组(173例)。结果治疗2年以后,4组患者的内皮素1(ET1)、线粒体动力相关蛋白1(DRP1)和线粒体分裂蛋白1(Fis1)水平均呈现明显下降的趋势,肱动脉血流介导的血管舒张功能(FMD)、NO、PTEN诱导假定激酶1(PINK1)、Parkin蛋白、微管相关蛋白1A/1B-轻链3(LC3)、线粒体融合蛋白1(Mfn1)、视神经萎缩蛋白1(OPA1)水平均上升。与所有的未调脂组比较,调脂组颈动脉内膜中层厚度(IMT)、DRP1、Fis1显著降低,PINK1、Parkin蛋白、LC3、Mfn1、OPA1显著增高(P<0.05)。校正混杂因素后,PINK1、Parkin蛋白、LC3、DRP1、Fis1、Mfn1和OPA1仍是IMT、FMD、ET1、NO的独立影响因素(P<0.01)。结论他汀调脂治疗通过改善线粒体自噬功能,可以改善老年高血压患者内皮功能紊乱。
Objective To investigate the effects of lipid-lowering therapy on endothelial function and mitophagy in the elderly patients with hypertension,and to explore the correlation of endothelial function with mitophagy.Methods A total of 672 patients who took physical examination in Cardiovascular and Cerebrovascular Disease Prevention and Research Center of Shandong Academy of Medical Sciences from April 2015 to November 2017 were selected as the study subjects.According to their blood pressure and plasma lipids and anti-hypertensive and/or lipid-lowering treatments,the participants were divided into hypertension without lipid-lowering group(n=135),hypertension combined with hyperlipidemia without lipid-lowering group(n=201),hypertension with lipid-lowering group(n=163),and hypertension combined with hyperlipidemia and lipid-lowering group(n=173).Results After two years’treatment,the systolic and diastolic blood pressure and plasma levels of ET1,DRP1,and Fis1 were decreased,whereas FMD and the plasma levels of NO,PINK1,Parkin,LC3,Mfn1 and OPA1 were increased in all four groups.Compared to the groups without lipid-lowering,the changes of IMT,DRP1,and Fis1 were significant lower,while PINK1,Parkin,LC3,Mfn1,and OPA1 were higher in the groups with lipid-lowering(P<0.05).After adjustment for confounders,the changes in PINK1,Parkin,LC3,DRP1,Fis1,Mfn1,and OPA1 were independent influencing factors for the changes in IMT,FMD,ET1 and NO(P<0.01).Conclusion Statin lipid-lowering therapy can ameliorate endothelial dysfunction in elderly hypertensive patients by improving mitophagy.
