摘要
目的探讨血清游离三碘甲状腺原氨酸(FT3)在成人脓毒症患者预后判断中的价值。方法选择2020年6月—2021年6月本院重症医学科收治的57名脓毒症患者作为研究对象,按28天死亡率分为存活组(n=39)和死亡组(n=18)两组。收集两组患者的临床特点及血清甲状腺激素水平,分析FT3与预后的相关性。结果入选者平均年龄(67.09±14.56)岁,28天病死率为31.6%,APACHEⅡ评分为(23.72±7.74);死亡组患者APACHEII评分、血肌酐、乳酸、序贯器官衰竭(SOFA)评分均高于存活组,而FT3水平低于存活组。多因素分析示FT3是成人脓毒症患者死亡的独立危险因素,FT3联合血乳酸(Lac)对成人脓毒症患者预后有更强的预测价值。结论成人脓毒症早期,FT3降低可能预示着患者预后不良,FT3联合血Lac早期预测成人脓毒预后具有重要的临床价值。
Objective To investigate the value of serum free triiodothyronine(FT3)in prognosis evaluating for adult sepsis patients.Methods A total of 57 patients suffered from sepsis those were admitted to intensive care unit of our hospital during June 2020 and June 2021 were enrolled as study objects.Patients were divided into survival group(n=39)and death group(n=18)according to 28-day mortality.Clinical characteristics and serum thyroid hormone levels of the patients were collected to analyze the correlation between FT3 and prognosis.Results The mean age of all patients was 67.09±14.56 years old,the 28-day mortality was 31.6%,and the APACHE II score was 23.72±7.74.The APACHE II score,serum creatinine,lactic acid,and sequential organ failure assessment(SOFA)score in the death group were higher than those in the survival group.The level of FT3 was lower than that of the survival group.Multi-factorial analysis showed that FT3 was an independent risk factor for death in adult sepsis patients,and FT3 combined with blood lactic acid(Lac)greatly improved the accuracy of predicting for the prognosis of adult sepsis patients.Conclusions In the early stage of sepsis in adult patients,reduced FT3 may indicate a poor prognosis,and FT3 combined with blood lactic acid(Lac)has important guiding significance in predicting the prognosis of adult sepsis.
