摘要
目的探讨疼痛管理配合穴位贴敷疗法在经尿道前列腺电切术(TURP)后下尿路症候群患者术后康复中的应用价值。方法选取2020年1月至2021年12月天长市中医院收治的86例TURP后下尿路症候群患者作为研究对象,根据1∶1随机数表法将患者分为观察组(n=43)和对照组(n=43)。对照组患者实施常规护理干预,观察组患者实施疼痛管理配合穴位贴敷干预。采用视觉模拟评分(VAS)法评估患者术后不同时间点的疼痛程度。于干预前及干预至出院时,比较两组患者前列腺症状(IPSS)评分、膀胱过度活动症状评分(OABSS)、匹兹堡睡眠质量指数量表(PSQI)评分、汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评分;并记录患者并发症发生率。结果术后6h、12h和24h观察组患者VAS评分均显著低于对照组(P<0.05);干预至出院时,观察组IPSS、OABSS、PSQI、HAMD和HAMA评分均显著低于对照组(P<0.05);观察组并发症发生率与对照组比较无明显差异(P>0.05)。结论疼痛管理配合穴位贴敷疗法在TURP后下尿路症候群患者术后康复中的应用价值较高,可降低患者疼痛程度,缓解前列腺症状和膀胱过度活动症状,改善心理状态,提高睡眠质量。
Objective To explore the application value of pain management combined with acupoint application in the rehabilitation of patients with lower urinary tract syndromes(LUTS)after transurethral resection of the prostate(TURP).Methods A total of 86 patients with LUTS after TURP admitted to Tianchang Hospital of Traditional Chinese Medicine were enrolled as the research objects between January 2020 and December 2021.According to random number table method,they were divided into observation group(n=43)and control group(n=43).The control group was given routine nursing,while observation group was given pain management combined with acupoint application.Pain degree was evaluated by scores of visual analogue scale(VAS).Before intervention and at discharge,International prostate symptom score(IPSS),overactive bladder symptom score(OABSS),scores of Pittsburgh Sleep Quality Index(PSQI),Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)were compared between the two groups.The incidence of complication was recorded.Results At 6h,12h and 24h after surgery,VAS scores in observation group were significantly lower than those in control group(P<0.05).From the intervention to discharge,IPSS,OABSS,PSQI,HAMD and HAMA scores in observation group were significantly lower than those in control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions Application value of pain management combined with acupoint application is high in the rehabilitation of patients with LUTS after TURP,which can relieve pain,prostate symptoms and overactive bladder,improve psychological state and sleep quality.
