摘要
目的评价他达那非片降阶梯治疗慢性前列腺炎合并勃起功能障碍的临床效果。方法选取2018年6月至2019年10月三亚中心医院诊治的120例Ⅲ型慢性前列腺炎合并勃起功能障碍患者作为研究对象。随机分为他达那非片组和常规治疗组,各60例。常规治疗组采用坦索罗辛及双石通淋中西医结合治疗;他达那非片组在常规治疗基础上加用他达那非片降阶梯治疗。比较治疗前及治疗后1、2、3个月两组前列腺炎症状指数(NIH-CPSI)、国际勃起功能指数-5(IIEF-5)、抑郁自评量表(SDS)和焦虑自评量表(SAS)评分,比较两组治疗后1、2、3个月的插入成功率、完成性交成功率。结果两组治疗后各个时间点NIH-CPSI评分较治疗前明显好转,差异具有统计学意义(P<0.05);常规治疗组治疗后1、2个月的IIEF-5、SDS、SAS评分与治疗前比较,差异无统计学意义(P>0.05),治疗后3个月与治疗前比较,差异具有统计学意义(P<0.05);他达那非片组治疗后各个时间点IIEF-5、SDS、SAS评分均较治疗前明显好转,且治疗后各个时间点IIEF-5评分明显优于同期常规治疗组,差异具有统计学意义(P<0.05);他达那非片组治疗后各个时间点插入成功率、完成性交成功率均明显优于同期常规治疗组,差异具有统计学意义(P<0.05)。结论他达那非片降阶梯治疗慢性前列腺合并勃起功能障碍能有效改善症状,调节患者心理状态,使患者重建信心,提高治疗耐受性及依从性。
Objective To evaluate the efficacy and safety of Tadalafil de-escalation therapy for chronic prostatitis with erectile dysfunction.Methods 120 patients with typeⅢchronic prostatitis complicated with erectile dysfunction treated in Sanya Central Hospital from June 2018 to October 2019 were selected as subjects.They were randomly divided into Tadanafil group and conventional treatment group,60 cases in each group.The conventional treatment group was treated with Tamsulosin and Shuangshi Tonglin.The Tadanafil group was treated with Tadanafil tablets on the basis of conventional treatment.The scores of the national institute of health-chronic prostatitis symptom index(NIH-CPSI),international erectile function index-5(IIEF-5),self-rating depression scale(SDS)and self-rating anxiety scale(SAS)were compared between the two groups before treatment and 1,2 and 3 months after treatment.The success rate of insertion and completion of intercourse after treatment were compared between the two groups 1,2 and 3 months after treatment.Results The NIH-CPSI scores of the two groups were significantly improved at each time point after treatment,and the difference was statistically significant(P<0.05).There was no significant difference in IIEF-5,SDS and SAS scores 1 and 2 months after treatment in conventional treatment group compared with before treatment(P>0.05),but there was significant difference 3 months after treatment compared with before treatment(P<0.05).The scores of IIEF-5,SDS and SAS in Tadanafil group were significantly improved at all time points after treatment compared with those before treatment,and the score of IIEF-5 at all time points after treatment was significantly better than that in conventional treatment group at the same period,with statistical significance(P<0.05).The success rate of insertion and completion of intercourse in Tadanafil group were significantly better than those in conventional treatment group at all time points after treatment,and the difference was statistically significant(P<0.05).Co
