针刺结合认知训练治疗脑卒中后认知障碍疗效Meta分析

摘要 目的:系统评价针刺结合认知训练治疗脑卒中后认知障碍的疗效,为临床治疗提供证据支持。方法:计算机检索PubMed、EMbase、Cochrane Library、中国知网、维普、万方及中国生物医学文摘等数据库,搜索关于针刺结合认知训练治疗脑卒中后认知障碍的随机对照试验,检索时间自建库起至2022年5月止。采用Cochrane协作网推荐的偏倚风险评估工具进行风险评估,用Jadad量表进行质量评价。RevMan5.2软件进行Meta分析,Begg’s检验分析发表偏倚。结果:最终纳入20篇研究文献(Jadad≥3),对纳入的1706例脑卒中后认知障碍(PSCI)患者进行Meta分析。针刺结合认知训练治疗在总有效率[RR=1.36,95%CI(1.15,1.59),P<0.05]、MMSE评分[SMD=1.22,95%CI(0.73,1.70),P<0.05]、MoCA评分[SMD=0.75,95%CI(0.35,1.16),P<0.05]、ADL评分[SMD=0.70,95%CI(0.39,1.00),P<0.05]改善程度上显著优于对照组患者,差异具有统计学意义。结论:针刺结合认知训练综合疗法在治疗脑卒中后认知障碍方面具有显著优势。 Objective:To systematically evaluate the efficacy of acupuncture combined with cognitive training in the treatment of cognitive impairment after stroke, and to provide evidence support for clinical treatment. Methods:PubMed, EMbase, Cochrane Library, CNKI,VIP,Wanfang and China Biomedical Abstracted databases were searched for randomized controlled trials on acupuncture combined with cognitive training in the treatment of cognitive impairment after stroke. The search time was from the establishment of the database to May 2022. The Cochrane Collaboration recommended bias risk assessment tool was used for risk assessment and the Jadad scale was used for quality assessment. Meta analysis was conducted by RevMan 5.2 software, and Begg’s test analysis was conducted for publication bias. Results:Twenty studies(Jadad≥3) were included in this meta-analysis, including 1706 patients with post-stroke cognitive impairment(PSCI). The total effective rate of acupuncture combined with cognitive training[RR=1.36,95%CI(1.15,1.59),P<0.05],MMSE score[SMD=1.22,95%CI(0.73,1.70),P<0.05],MoCA score[SMD=0.75,95%CI(0.35,1.16),P<0.05],ADL score[SMD=0.70,95%CI(0.39,1.00),P<0.05] were significantly better than the control group, and the difference was statistically significant. Conclusion:Acupuncture combined with cognitive training has significant advantages in the treatment of cognitive impairment after stroke.
