摘要
目的探讨高同型半胱氨酸血症合并脑梗死患者动脉粥样硬化与血清尿酸(UA)及脂蛋白a[Lp(a)]的关系。方法选取锦州医科大学附属第一医院神经内科2021年8月至2022年5月住院的急性脑梗死患者442例,入院后根据血浆同型半胱氨酸(Hcy)的水平高低分为实验组(Hcy≥10μmol/L,n=274)和对照组(Hcy<10μmol/L,n=168),两组患者抽取空腹静脉血测定血清UA、Lp(a)水平,并应用多普勒彩超测定颈动脉内膜中层厚度(IMT)及斑块情况,比较其动脉粥样硬化程度与血清UA、Lp(a)的相关性。结果实验组患者的血清UA、Lp(a)水平均高于对照组(P<0.05);实验组斑块形成的比例高于对照组,差异均有统计学意义(P<0.05)。经Pearson相关性分析显示高同型半胱氨酸血症合并脑梗死患者IMT值与UA、Lp(a)水平呈正相关(P<0.05)。Logistic回归分析显示血清UA、Lp(a)是导致实验组动脉内膜增厚及斑块形成的独立危险因素(P<0.05)。结论高同型半胱氨酸血症合并脑梗死患者血清UA、Lp(a)水平升高,血清UA、Lp(a)是导致其动脉粥样硬化病情进展的独立危险因素。
【objective】To investigate the relationship between atherosclerosis and serum UA and Lp(a)in patients with hyperhomocysteinemia complicated with cerebral infarction.【Methods】A total of 442 patients with acute cerebral infarction admitted in the Department of Neurology of the First Affiliated Hospital of Jinzhou Medical University from August 2021 to May 2022 were divided into experimental group(Hcy≥10μmol/L)and control group(Hcy<10μmol/L).Fasting venous blood samples were taken from the two groups to determine the levels of serum UA and Lp(a).The intima-media thickness(IMT)and plaque of carotid artery were measured by Doppler ultrasound,and the correlation between the degree of atherosclerosis and serum UA and Lp(a)was compared.【Results】The levels of serum UA and Lp(a)in the experimental group were higher than those in the control group(P<0.05).The proportion of plaque formation in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that IMT value was positively correlated with UA and Lp(a)levels in patients with hyperhomocysteinemia complicated with cerebral infarction(P<0.05).Logistic regression analysis showed that serum UA and Lp(a)were independent risk factors for intimal thickening and plaque formation in the experimental group(P<0.05).【Conclusion】The levels of serum UA and Lp(a)are increased in patients with hyperhomocysteinemia complicated with cerebral infarction.Serum UA and Lp(a)are independent risk factors for the progression of atherosclerosis.
