摘要
目的 探讨磁共振T2WI信号强度对鉴别外周带前列腺癌及慢性前列腺炎的诊断效能.方法 选取2018年4月至2020年11月在本院行穿刺活检确诊的32例外周带前列腺癌患者(外周带前列腺癌组)和15例慢性前列腺炎患者(慢性前列腺炎组)的临床资料.采集患者临床及实验室指标,观察磁共振T2WI序列上病灶形态、测量同一层面病变与闭孔内肌信号强度的比值,确定鉴别诊断的临界值,并评价该试验方法的诊断效能.结果 外周带前列腺癌组以单侧发生、不均质、边界模糊的T2WI低信号结节或肿块灶多见,慢性前列腺炎组以双侧发生、不均质、边界模糊的斑片状T2WI低信号多见.两组在病灶发生的侧别和病变形态上比较,差异均有统计学意义(均P<0.05).外周带前列腺癌组的T2WI信号强度比值低于慢性前列腺炎组,差异有统计学意义[(2.17±0.524)vs.(2.97±0.759),P=0.031].两组研究对象的T2WI信号强度的受试者工作特征曲线下面积为0.816,鉴别诊断的界值为2.25,该临界值的诊断特异度为92.3%,灵敏度为61.3%.结论 测定T2WI信号强度对于鉴别外周带前列腺癌及慢性前列腺炎具有一定帮助.
ObjectiveTo explore the diagnostic efficacy of MRI T2WI signal intensity in distinguishing peripheral prostate cancer from prostatitis.Methods From April 2018 to November 2020,32 cases of peripheral prostate cancer(peripheral prostate cancer group)and 15 cases of chronic prostatitis(chronic prostatitis group)confirmed by needle biopsy of our hospital were collected.Clinical and laboratory indicators were collected and magnetic resonance imaging was observed.The shape of the lesion on the T2WI sequence,the ratio of the signal intensity between the lesions at the same level and the obturator internal muscle were measured,ROC curve was drawn and critical value of the differential diagnosis was determined,while diagnostic efficacy of the experimental method was evaluated.Resulttss Peripheral prostate cancer was more common with unilateral,heterogeneous,and fuzzybounded T2WI low signal nodules or masses,and prostatitis was more common with bilateral,heterogeneous,and blurred T2WI patchy low signal.There were significant differences between the two groups in the side of the lesion and the morphology of the lesion(all P<0.05).The T2WI signal intensity ratio of peripheral prostate cancer group was lower than that of chronic prostatitis group,and the difference was statistically significant[(2.17±0.524)vs.(2.97±0.759),P=0.031].The area under the curve of the ROC curve was 0.816,the cutoff value for differential diagnosis was 2.25,the diagnostic specificity of this cutoff value was 92.3%,and the sensitivity was 61.3%.Conclusions Analyzing and measuring the signal intensity of T2WI is helpful to distinguish peripheral prostate cancer and chronic prostatitis.
