摘要
目的探讨桂枝汤加减联合穴位贴敷治疗脾胃虚弱型妊娠剧吐的疗效,以冀为制订临床决策提供参考。方法选取2021年11月—2022年5月在本院住院诊治的脾胃虚弱型妊娠剧吐孕妇60例作为研究对象,根据随机数字表法将患者分为中医组与对照组,每组30例。对照组患者给予西医补液治疗,中医组患者在对照组的基础上,给予桂枝汤加减联合穴位贴敷治疗,2组患者均治疗观察2周,比较2组患者的疗效、生活质量等。结果治疗后,中医组患者的总有效率为100.0%,高于对照组的86.7%,但差异并无统计学意义(P>0.05)。中医组患者的止吐时间、住院时间、尿酮体转阴时间、电解质恢复正常时间都显著短于对照组,差异均有统计学意义(P<0.05)。治疗前,2组患者恶心呕吐指数问卷(RINV)得分比较,差异无统计学意义(P>0.05);治疗后,2组患者RINV得分均低于治疗前,且中医组显著低于同期对照组,差异均有统计学意义(P<0.05)。治疗后,中医组患者在生活质量综合评定量表各维度上的得分均高于对照组,差异均有统计学意义(P<0.05)。结论桂枝汤加减联合穴位贴敷治疗脾胃虚弱型妊娠剧吐能有效改善孕妇恶心呕吐等临床症状,并提高孕妇生活质量。
Objective To investigate the therapeutic effect of Guizhi decoction combined with acupoint application on hyperemesis gravidarum of spleen-stomach weakness type to provide guidance for the decision-making of clinical treatment.Methods Sixty pregnant women with hyperemesis gravidarum of spleen-stomach weakness type who were hospitalized in our hospital from November 2021 to May 2022 were selected as the study participants.They were assigned to a TCM group(n=30)and a control group(n=30)using a random number table method.Patients in the control group received western medicine fluid replacement therapy,and patients in the TCM group received Guizhi decoction combined with acupoint application based on the control group.The patients in both groups were treated and observed for 2 weeks.The efficacy and quality of life of the patients in the two groups were compared.Results After treatment,the overall response rate of the TCM group was 100.0%,which was higher than that of the control group(86.7%),but the difference was not statistically significant(P>0.05).The antiemetic time,hospital stay,urine ketone body conversion time,and electrolyte recovery time of patients in the TCM group were significantly shorter than those in the control group(P<0.05).Before treatment,there was no significant difference in the scores of the Radiation-Induced Nausea and Vomiting(RINV)questionnaire between the two groups(P>0.05);after treatment,the scores of RINV in the two groups were lower than those before treatment,and the TCM group was lower than the concurrent control group,with statistical significance(P<0.05).After treatment,the subscales scores of the comprehensive quality of life rating scale in the TCM group were significantly higher than those in the control group(P<0.05).Conclusion Guizhi decoction combined with acupoint application in the treatment of hyperemesis gravidarum of spleen-stomach weakness type can improve the symptoms of nausea and vomiting,increase the quality of life of pregnant women.
