摘要
目的分析心脏磁共振T1 mapping技术在肥厚型心肌病(HCM)心肌纤维化中定量参数改变及诊断效果。方法选取自2020年2月至2022年3月接收的51例肥厚型心肌病患者为观察组,同期选取49例健康志愿者为对照组。两组患者均进行心脏磁共振检查,比较两组患者的心功能参数,记录各段心肌增强前、后的T1值、ECV,比较不同心脏磁共振(CMR)参数对HCM的诊断价值。结果观察组SV、LVEF及LVSVI均高于对照组(P<0.05);观察组心肌质量、MMI、最大室壁厚度及间隔壁厚度均大于对照组(P<0.05)。观察组心肌增强前T1值及ECV均高于对照组(P<0.05),且肥厚段心肌高于非肥厚段(P<0.05)。LGE阳性患者整体心肌和LGE阴性患者肥厚段心肌的增强前T1值和ECV均高于对照组(P<0.05)。LGE阴性患者肥厚段心肌的增强前T1值和ECV均高与非肥厚段(P<0.05)。增强前T1值、增强后T1值、ECV的灵敏度分别为84.32%、76.49%、85.72%,特异度分别为67.31%、63.38%、65.34%,增强前T1值AUC=0.801(95%CI:0.712~0.890),增强后T1值AUC=0.774(95%CI:0.677~0.870),ECV的AUC=0.814(95%CI:0.730~0.897)。结论心脏磁共振T1 mapping技术在HCM心肌纤维化中的诊断效果显著,能较早的发现HCM患者心肌纤维化,且ECV参数具有较高的诊断效能。
Objective To analyze the change of quantitative parameters and its diagnostic effect of cardiac magnetic resonance T1 mapping in HCM myocardial fibrosis.Methods 51 patients with hypertrophic cardiomyopathy enrolled from February 2020 to March 2022 were selected as the observation group,and 49 healthy volunteers during the same period were selected as the control group.The patients in the two groups underwent cardiac magnetic resonance examination,then the cardiac function parameters of patients in the two groups were compared.The T1 value and ECV before and after myocardial enhancement were recorded,and the diagnostic value of different CMR parameters for HCM was compared.Results SV,LVEF and LVSVI in the observation group were higher than those in the control group(P<0.05).Moreover,the myocardial mass,MMI,maximum ventricular wall thickness and septal wall thickness in the observation group were higher than those in the control group(P<0.05).T1 value and ECV before myocardial enhancement in the observation group were higher than those in the control group(P<0.05),and the hypertrophic myocardium is also higher than the non hypertrophic myocardium(P<0.05).The pre-enhanced T1 value and ECV of the whole myocardium of the LGE-positive patients and the hypertrophic segment of the LGE-negative patients were higher than those of the control group(P<0.05).The pre-enhanced T1 value and ECV of hypertrophic segment of LGE-negative patients were higher than those of non-hypertrophic segment(P<0.05).Finally,the sensitivity of T1 value before enhancement,T1 value after enhancement and ECV were 84.32%,76.49%and 85.72%,respectively,and the specificity were 67.31%,63.38%and 65.34%,respectively.In addition,the T1 value before enhancement was AUC=0.801(95%CI:0.712-0.890),the T1 value AUC after enhancement was 0.774(95%CI:0.677-0.870),as well as the AUC of ECV was 0.814(95%CI:0.730-0.897),respectively.Conclusion Cardiac magnetic resonance T1 mapping technology has a significant diagnostic effect on myocardial fibrosis in hypertrophic
