摘要
目的:探讨可溶性CD86(sCD86)、高迁移率族蛋白B1(HMGB1)在过敏性鼻炎患儿外周血中表达及临床意义。方法:选取过敏性鼻炎患儿150例作为观察组,按照病情严重程度分为轻度组74例和中重度组76例;另选择同期健康体检的正常儿童50名作为对照组。比较各组免疫球蛋白E(IgE)、白细胞介素(IL)-4、IL-9、干扰素-γ(IFN-γ)、sCD86、HMGB1水平,并对观察组患儿以上指标进行多因素分析和相关性分析。结果:观察组IgE、IL-4、IL-9、sCD86、HMGB1水平均明显高于对照组,IFN-γ水平明显低于对照组(P<0.01)。中重度组患儿IgE、IL-4、IL-9、sCD86、HMGB1水平均明显高于轻度组,IFN-γ水平明显低于轻度组(P<0.01)。IgE、IFN-γ、IL-4、IL-9、sCD86及HMGB1均为过敏性鼻炎患儿病情严重程度的独立影响因素(P<0.05~P<0.01)。观察组患儿血清sCD86、HMGB1水平与IgE、IL-4、IL-9均呈正相关关系(P<0.05),与IFN-γ均呈负相关关系(P<0.05)。结论:过敏性鼻炎患儿血清中sCD86、HMGB1水平呈高表达,其原因可能与IgE介导的Ⅰ型变态反应及Th1/Th2细胞失衡有关。
Objective:To explore the expression and clinical significance of soluble CD86(sCD86)and high mobility group protein B1(HMGB1)in peripheral blood of children with allergic rhinitis.Methods:A total of 150 children with allergic rhinitis were selected as the observation group.They were divided into mild group(n=112)and moderate to severe group(n=38)according to the severity of the disease,and 50 normal children who underwent physical examination in the same period were selected as the control group.The levels of immunoglobulin E(IgE),interleukin-4(IL-4),interleukin-9(IL-9),interferon-γ(IFN-γ),sCD86 and HMGB1 in each group were compared.In the observation group,the sCD86,HMGB1 levels and IgE,IL-4,IL-9,IFN-γlevels were analyzed by univariate,logistic multivariate and correlation analysis.Results:IgE,IL-4,IL-9,sCD86 and HMGB1 levels in the observation group were significantly higher than that in the control group;IFN-γlevels were significantly lower than that in the controls group(P<0.01).Moreover,IgE,IL-4,IL-9,sCD86 and HMGB1 levels in the moderate to severe group were significantly higher than those in the mild group;IFN-γlevels were significantly lower than that in the mild group(P<0.01).IgE,IFN-γ,IL-4,IL-9,sCD86 and HMGB1 were the independent influencing factors for the severity of allergic rhinitis(P<0.05 to P<0.01).In sera from children with allergic rhinitis,sCD86 and HMGB1 levels were positively correlated with IgE,IL-4 and IL-9(P<0.05),and negatively correlated with IFN-γ(P<0.05).Conclusions:The high expression of sCD86 and HMGB1 in serum of children with allergic rhinitis may be related to IgE mediated typeⅠallergy and Th1/Th2 cell imbalance.
