摘要
目的:探讨输尿管软镜治疗肾结石的效果及对患者肾功能的影响。方法:选取2019年6月—2022年6月北京市普仁医院收治的240例肾结石患者作为研究对象,所有患者采用输尿管软镜碎石术,观察患者手术时间、术中出血量、住院时间、结石清除率、并发症发生情况以及手术前后血清肌酐(Scr)、半胱氨酸蛋白酶抑制剂C(Cys-C)、中性粒细胞明胶酶相关载脂蛋白(NGAL)水平。结果:患者手术时间为(92.18±1.91)min,术中出血量为(12.56±2.04)mL,住院时间为(4.57±1.58)d。1期结石清除208例,2期结石清除16例,结石清除率为93.33%。术后出现发热4例。患者术后2 h、12 h Scr、Cys-C、NGAL水平均高于术前,差异有统计学意义(P<0.05);术后72 h,患者Scr、Cys-C、NGAL水平与术前比较,差异无统计学意义(P>0.05)。结论:输尿管软镜碎石术操作简单,对患者创伤小,虽然对肾功能有一定的损伤,但损伤程度较轻,且恢复快,建议应用。
Objective:To investigate the effect of flexible ureteroscopy in the treatment of renal calculi and its influence on renal function of patients.Methods:A total of 240 patients with renal calculi admitted to Beijing Puren Hospital from June 2019 to June 2022 were selected as the study subjects.And all patients were treated with flexible ureteroscopic lithotripsy.The operation time,intraoperative blood loss,length of hospital stay,stone clearance rate,complications,serum creatinine(Scr),cystatin C(Cys-C),and neutrophil gelatinase-associated lipocalin(NGAL)levels before and after surgery were observed.Results:The operation time was(92.18±1.91)min,the intraoperative blood loss was(12.56±2.04)mL,and the length of hospital stay was(4.57±1.58)d.There were 208 cases of stone clearance at stage 1 and 16 cases of stone clearance at stage 2,and the stone clearance rate was 93.33%.Four cases developed fever after surgery.The levels of Scr,Cys-C and NGAL at 2 h and 12 h after surgery were higher than those before surgery,and the differences were statistically significant(P<0.05).At 72 h after surgery,the levels of Scr,Cys-C and NGAL showed no significant difference compared with those before surgery(P>0.05).Conclusion:Flexible ureteroscopic lithotripsy has simple operations,less trauma on patients.Although it induces some damage to renal function,the damage severity is mild and the recovery is fast,which is recommended for application.
