摘要
目的观察孙氏选穴针刺法联合桂香温经止痛胶囊治疗寒凝血瘀型子宫内膜异位症(EMs)痛经的临床疗效。方法将80例寒凝血瘀型EMS痛经患者按照随机数字表法分为2组,对照组40例予桂香温经止痛胶囊治疗,治疗组40例在对照组基础上联合孙氏选穴针刺法治疗。2组均治疗3个月经周期后统计疗效,比较2组治疗前后疼痛视觉模拟评分(VAS)、中医证候(包括小腹冷痛、腰骶酸胀痛、畏寒肢冷、月经后期、经血量少、经色暗有血块、便溏、小便清长、舌质暗有瘀斑、脉沉涩)评分及血清前列素E_(2)(PGE_(2))水平变化情况。结果治疗组总有效率87.50%(35/40),对照组总有效率72.50%(29/40),治疗组疗效优于对照组(P<0.05)。与本组治疗前比较,2组治疗后疼痛VAS均降低(P<0.05),且治疗组治疗后疼痛VAS低于对照组(P<0.05)。与本组治疗前比较,2组治疗后中医证候小腹冷痛、腰骶酸胀痛、畏寒肢冷、月经后期、经血量少、经色暗有血块、便溏、小便清长、舌质暗有瘀斑、脉沉涩评分及总分均降低(P<0.05),且治疗组治疗后中医证候各项评分及总分均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后血清PGE_(2)水平均降低(P<0.05),且治疗组治疗后血清PGE_(2)水平低于对照组(P<0.05)。结论孙氏选穴针刺法联合桂香温经止痛胶囊治疗寒凝血瘀型EMS痛经疗效显著,可有效缓解患者疼痛症状,改善中医证候表现,其作用机制可能与降低血清PGE_(2)水平有关。
Objective To observe the therapeutic effect of Professor Sun's acupoint selection acupuncture on endometriosis dysmenorrhea with cold coagulation and blood stasis type.Methods Totally 80 patients with endometriosis dysmenorrhea(cold coagulation and blood stasis type)were randomized 1∶1 to receive Guixiang Wenjing Zhitong Capsule(the control group)or Sun's acupoint selection acupuncture+Guixiang Wenjing Zhitong Capsule(the treatment group),the efficacy was observed after 3 menstrual cycles.The Visual analogue scale(VAS),the traditional Chinese medicine(TCM)syndrome factors(hypogastrium cold pain,lumbosacral pain,chills,cold limbs,late menstruation,less menstrual blood,dark color with blood clots,loose stools,clear urine,dark tongue with ecchymosis,deep and astringent pulse),and changes in serum prostaglandin E_(2)(PGE_(2))levels were observed.Results After treatment,the total effective rate in the treatment group was better than that in the control group,(87.50%[35/40]vs 72.50%[29/40],P<0.05,respectively).Compared with those before treatment,the VAS in groups was decreased after treatment(P<0.05),and which in the treatment group was lower than that in the control group(P<0.05).The scores and total scores of TCM syndromes in groups were decreased(P<0.05),and which were lower in the treatment group than in the control group(P<0.05).The serum levels of PGE_(2) in groups were decreased(P<0.05),improvement of which in the treatment group was better compared with the control group(P<0.05).Conclusion For patients with endometriosis dysmenorrhea(cold coagulation and blood stasis type),the combination has a significant effect,it can effectively relieve pain and improve TCM syndromes by inhibiting serum PGE_(2) levels.
