摘要
目的推进前置审方,实现审方闭环管理,促进合理用药。方法在前置审方的推进中,针对审方工作模式的建立、审核系统中知识库规则的完善、拦截功能及自动干预等问题,实施三次阶段性改进。通过比较运行前置审方系统后一年的门诊及住院药房不合理处方/医嘱率,评价该系统的运行效果。结果通过定期修改知识库规则,前置审方工作在不断改进中推行,初次不合理处方率明显降低。经过三次改进后,全院门诊处方初次不合理率降幅为72.56%,全院住院医嘱初次不合理率降幅为63.61%。结论前置审方在不断改进中推行,实现审方闭环管理,保障全院合理用药。
Objective To promote the pre-prescription audit system to realize the closed-loop management of the prescription audit,and to improve the rational use of drugs.Methods In the promotion of the pre-prescription audit,three phased improvements were implemented for the establishment of the working mode of the prescription audit,the improvement of the knowledge base rules in the audit system,and the intercepting function and automatic intervention.The operative effects of the pre-prescription audit system was evaluated by evaluating the initial unreasonable rate of prescriptions/orders in outpatient and inpatient pharmacies after the operation of the pre-prescription audit system.Results After three improvements,the initial unreasonable rate of outpatient prescriptions decreased by 72.56%,and the initial unreasonable rate of inpatient medical orders decreased by 63.61%.Conclusion The pre-prescription audit system is implemented in the continuous improvement,in order to realize the closed-loop management of the prescription audit,and ensure the rational use of drugs in the whole hospital.
