摘要
目的评价透脓散加减方治疗小儿穿孔性阑尾炎合并腹膜炎术后腹腔感染的效果。方法随机将87例穿孔性阑尾炎伴腹膜炎术后腹腔感染患儿分为治疗组56例、对照组31例,所有患儿中医辨证均为脾虚热腐成脓证,对照组予抗感染治疗,治疗组在抗感染治疗基础上口服透脓散加减方,2组疗程均为1周。通过观察2组疗效、脓肿吸收情况,评价透脓散加减方对小儿穿孔性阑尾炎合并腹膜炎术后腹腔感染的治疗效果。结果疗效方面,治疗组总有效率为91.1%(51/56),对照组总有效率为74.2%(23/31),治疗组优于对照组(P<0.05)。在脓肿吸收方面,治疗组治疗后脓肿完全吸收或者缩小的共51例,脓肿未吸收或者增大的5例;对照组治疗后脓肿完全吸收或者缩小的共23例,脓肿未吸收或者增大的8例。结论对于小儿穿孔性阑尾炎合并腹膜炎术后腹腔感染,透脓散加减方可提高总有效率,改善相关临床症状,促进腹腔脓肿的吸收或缩小。
Objective To evaluate the effect of Tounong powder on abdominal infection after operation of perforated appendicitis complicated with peritonitis in children.Methods 87 children with abdominal infection after operation of perforated appendicitis with peritonitis were randomly divided into treatment group(56 cases)and control group(31 cases).All children had syndrome of spleen deficiency heat rotting into pus according to traditional Chinese medicine syndrome differentiation.The control group was treated with anti infection treatment,and the treatment group took Tounong powder on the basis of anti infection treatment.The course of treatment of both groups was 1 week.By observing the curative effect and abscess absorption of the two groups,the therapeutic effect of Tounong powder on abdominal infection after operation of perforated appendicitis complicated with peritonitis in children was evaluated.Results The total effective rate was 91.1%(51/56)in the treatment group and 74.2%(23/31)in the control group,which was better than that in the control group(P<0.05).In terms of abscess absorption,51 cases of abscess were completely absorbed or reduced after treatment,and 5 cases of abscess were not absorbed or increased.In the control group,23 cases of abscess were completely absorbed or reduced after treatment,and 8 cases of abscess were not absorbed or enlarged.Conclusion For abdominal infection after operation of children with perforated appendicitis complicated with peritonitis,Tounong powder can improve the total effective rate,improve relevant clinical symptoms,and promote the absorption or reduction of abdominal abscess.
