摘要
目的探讨T2DM患者残余胆固醇(RC)与代谢相关脂肪性肝病(MAFLD)的相关性。方法选取2020年1月至2021年12月于福建医科大学附属三明第一医院内分泌科住院治疗的976例T2DM患者,分为合并MAFLD组(MAFLD,n=563)和单纯T2DM组(T2DM,n=413)。比较两组一般资料及生化指标,Logistic回归分析MAFLD的影响因素,受试者工作特征(ROC)曲线分析RC、RC联合BMI、谷丙转移酶(ALT)对MAFLD的预测价值。结果与T2DM组比较,MAFLD组年龄、DM病程、HDL-C降低(P<0.01),BMI、SBP、DBP、FPG、TC、TG、SUA、ALT、谷草转移酶、血清白蛋白(ALB)、γ-谷氨酰转肽酶、RC升高(P<0.05或P<0.01)。按照RC四分位数分组,各组MAFLD的检出率分别为34.07%、55.43%、61.13%、78.37%(P<0.01)。Logistic回归分析结果显示,RC是MAFLD的影响因素。ROC曲线分析显示,RC曲线下面积(AUC)为0.688,RC联合BMI、ALT的AUC为0.788。结论RC是T2DM患者合并MAFLD的影响因素,对MAFLD有预测价值。
Objective To investigate the correlation between residual cholesterol(RC)and metabolic associated fatty liver disease(MAFLD)in patients with type 2 diabetes mellitus(T2DM).Methods 976T2DM patients hospitalized in the Department of Endocrinology,Sanming First Hospital Affiliated to Fujian Medical University from January 2020 to December 2021 were selected and divided into MAFLD group(n=563)and simple T2DM group(T2DM,n=413)according to the presence of combined MAFLD.The general information and biochemical indicators were compared between the two groups.Logistic regression analysis was used to analyze the influencing actors of MAFLD.Receiver operating characteristic curve(ROC)was used to analyze the predictive value of RC,RC in combination with BMI and ALT for MAFLD.Results Compared with the T2DM group,the age,DM duration and HDL-C in the MAFLD group were decreased(P<0.01),while BMI,SBP,DBP,FPG,TC,TG,SUA,ALT,AST,ALB,GGT,and RC were increased(P<0.05 or P<0.01).According to the RC quartile level,the detection rates of MAFLD from low RC group to high RC group were 34.07%,55.43%,61.13%and 78.37%(P<0.01).Logistic regression analysis showed that RC was an influencing factor of MAFLD.ROC analysis showed that the area under the curve(AUC)of RC was 0.688,and the AUC of RC combined with BMI and ALT was 0.788.Conclusion RC is a risk factor for MAFLD with predictive value in T2DM patients.
