摘要
目的探讨前置审方流程对天门市第一人民医院特殊患者不合理用药的影响。方法收集合理用药软件系统对天门市第一人民医院2021年9-12月的儿科、妇科、产科医嘱审核的监测数据,对其进行统计和分析。结果合理用药软件系统监测医嘱2021年9-10月干预前组有4836例,不合理率2.34%;2021年11-12月干预后组有5018例,不合理率为0.60%,差异有统计学意义(P<0.05)。不合理用药类型主要有给药剂量、频次、途径不合理,特殊人群(如孕妇禁用,儿童慎用)不宜使用仍选用,配伍不合理,无适应证用药,以及其他(含重复用药)等。结论合理用药软件系统与药师审方相结合的处方前置审核模式,可有效降低不合理医嘱发生率,提高合理用药水平,保证孕产妇和儿童用药安全。
Objective To explore the influence of the process of pre-audit of prescriptions on the irrational drug use of special patients in the Tianmen First People′s Hospital.Methods The monitoring data of pre-audit of prescriptions in pediatric,gynecologic and obstetrical departments in Tianmen First People′s Hospital from September to December in 2021 were collected and analyzed by using rational drug use software system.Results There were 4836 cases in pre-intervention group in the rational drug use software monitoring system from September to October in 2021,with an unreasonable rate of 2.34%.There were 5018 cases from November to December in 2021 in intervention group,the unreasonable rate being 0.60%,the difference was statistically significant(P<0.05).The types of irrational use of drugs mainly included the unreasonable dosage,frequency and route,special population(such as forbidden for pregnant women,cautious for children),unreasonable combination,no indication,others(including repeated use of drugs)and so on.Conclusion The combination of rational drug use software system and pharmacist′s prescription pre-audit mode can effectively reduce unreasonable medical advice,improve the level of rational drug use,and ensure the safety of drug use for pregnant women and children.
