摘要
目的基于实验室信息系统(LIS)中健康体检者甲状腺激素检验数据,采用间接法建立本地50岁及以上人群FT_(3)和FT_(4)参考区间。方法从LIS中选取2019年4月至8月期间50岁以上体检人群甲状腺激素检测数据,剔除极端值后,Box-Cox将数据转换为正态分布后再用Turkey法剔除离群值,经K-S检验正态分布的数据以x±1.96 s建立参考区间,非正态分布数据用P_(2.5)、P_(97.5)建立参考区间,并基于生物学变异参考变化值(RCV)来验证本研究建立参考区间的可靠性。结果FT_(3)在性别间和甲状腺自身免疫抗体差异无统计学意义(P>0.05);FT_(3)在各年龄组间呈正态分布,年龄组间差异有统计学意义(P<0.01),50~60、61~70、71~75、≥76岁组95%置信区间(95%CI)分别为3.05~5.79、2.89~5.67、2.78~5.40、2.52~5.26pmol/L,FT_(3)随年龄增加呈下降趋势。性别、不同年龄组间FT_(4)差异无统计学意义(P>0.05);FT_(4)在不同甲状腺自身免疫抗体水平差异有统计学意义(P<0.01),TpoAb和TgAb正常组(≤10kU/L)FT_(4)数据呈偏态分布,参考区间为9.51~17.97pmol/L。本研究建立的FT_(3)和FT_(4)参考区间与试剂说明书参考区间变化值在RCV内。结论间接法适用于50岁以上人群FT_(3)和FT_(4)参考区间的建立,性别对FT_(3)和FT_(4)无影响,FT_(3)随着年龄增加呈下降趋势。
Objective Based on the thyroid function test data of physical examinees in laboratory information system(LIS),to establish FT_(3)and FT_(4)reference intervals of over 50 years old people by indirect method.Methods We selected the thyroid hormone test data of physical examination population over 50 years old from LIS,from April to August,2019.After excluding the extreme value,Box-Cox method was used to convert the data into a normal distribution,and then Turkey method was applied to eliminate the outliers.With K-S test,the reference interval was established with x±1.96 s for normal distribution data,and P_(2.5)and P_(97.5)for skew distribution data.The reliability of the reference interval established in this study was verified based on the biological variation reference change value(RCV).Results There was no significant difference in FT_(3)between sex and different thyroid antibody levels;FT_(3)was normally distributed among all age groups,and the difference was statistically significant.The 95%confidence interval(95%CI)of 50-60 years old,61-70 years old,71-75 years old and≥76 years old groups were 3.05-5.79 pmol/L,2.89-5.67pmol/L,2.78-5.40 pmol/L and 2.52-5.26 pmol/L,respectively.FT_(3)decreased with age.There was no significant difference in FT_(4)between gender and different age groups;FT_(4)had statistical significance in different thyroid antibody levels.FT_(4)data of TPOAb and TGAb normal group(≤10 kU/L)showed a skewed distribution,and the reference interval was 9.51-17.97pmol/L.The change values of FT_(3)and FT_(4)reference intervals established in this study and the reference interval of reagent instructions were within RCV.Conclusion Indirect method is applicable to the establishment of FT_(3)and FT_(4)reference interval.Gender has no effect on FT_(3)and FT_(4).FT_(3)shows a downward trend with age.
