摘要
目的探讨S100钙结合蛋白A8(S100A8)、晚期糖基化终末产物受体(RAGE)表达水平变化与新生儿呼吸窘迫综合征(NRDS)疾病严重程度及预后的关系。方法选取2019年9月至2021年9月本院收治的102例NRDS患儿为NRDS组,同期健康新生儿102例为对照组。收集整理患儿性别、出生日龄、体重及致病原因等临床资料。采用酶联免疫吸附试验(ELISA)检测血清S100A8、RAGE水平;NRDS患儿血清S100A8、RAGE水平与SNAPPE-II评分的相关性采用Pearson法分析;多因素Logistic回归分析影响患儿预后的可能因素;S100A8、RAGE对患儿预后的预测价值采用ROC曲线评估。结果NRDS组患儿血清S100A8、RAGE水平较对照组均明显升高,差异有统计学意义(P<0.05);NRDS患儿病情越严重,血清S100A8、RAGE水平及SNAPPE-II评分就越高,且重度组患儿血清S100A8、RAGE水平及SNAPPE-II评分显著高于轻度组(P<0.05),Ⅳ级NRDS患儿血清RAGE水平和SNAPPE-II评分显著高于Ⅲ级NRDS患儿(P<0.05);NRDS患儿血清S100A8、RAGE水平与SNAPPE-II评分均呈显著正相关(r=0.566、0.508,P<0.05);生存组NRDS患儿血清S100A8、RAGE水平均显著低于死亡组(P<0.05);ROC曲线显示,血清中S100A8对NRDS患儿预后预测的曲线下面积(AUC)为0.833,截断值为192.49pg/mL,其灵敏度、特异性分别为84.21%、75.90%;RAGE对NRDS患儿预后预测的AUC为0.889,截断值为22.48μg/mL,其灵敏度、特异性分别为78.95%、87.95%;二者联合对NRDS患儿预后预测的AUC为0.935,其灵敏度、特异性分别为78.95%、96.39%;SNAPPE-II评分在生存组和死亡组NRDS患儿间差异具有统计学意义(P<0.05);多因素Logistic回归分析显示S100A8、RAGE、SNAPPE-II评分均是影响NRDS患儿预后的危险因素(P<0.05)。结论血清S100A8、RAGE在NRDS患儿中高表达,二者均与疾病严重程度显著相关,且可能用于NRDS患儿的预后评估。
Objective To investigate the relationship between the expression levels of S100 calcium-binding protein A8(S100A8)and receptor for advanced glycation end products(RAGE)and the severity and prognosis of neonatal respiratory distress syndrome(NRDS).Methods From September,2019 to September,2021,102 children with NRDS admitted to our hospital were selected as the NRDS group,while 102 healthy newborns were included as the control group.Clinical data,such as gender,age at birth,body weight,and cause of disease,were collected and processed.The levels of serum S100A8 and RAGE were detected by enzyme-linked immunosorbent assay(ELISA);The correlation between serum S100A8 and RAGE levels and SNAPPE-II score in children with NRDS was analyzed by Pearson method;Multivariate logistic regression was developed to analyze the potential factors affecting the prognosis of children;The predictive value of S100A8 and RAGE on the prognosis of children was evaluated by ROC curve.Results The levels of serum S100A8 and RAGE in the NRDS group were greatly higher than those in the control group(P<0.05);The more severe the condition of the NRDS children,the higher the serum S100A8 and RAGE levels and the SNAPPE-II score;The serum S100A8,RAGE levels and SNAPPE-II score of the children in the severe group were greatly higher than those in the mild group(P<0.05);The serum RAGE level and SNAPPE-II score of children with gradeⅣNRDS were greatly higher than those of children with gradeⅢNRDS(P<0.05);The levels of serum S100A8 and RAGE in children with NRDS were positively correlated with SNAPPE-II score(r=0.566,0.508,P<0.05);The levels of serum S100A8 and RAGE in children with NRDS in the survival group were greatly lower than those in the death group(P<0.05);ROC curve showed that the area under the curve(AUC)of serum S100A8 for predicting the prognosis of children with NRDS was 0.833,while the cut-off value was 192.49 pg/mL,and its sensitivity and specificity were 84.21%and 75.90%,respectively;AUC of RAGE for predicting the prognosis of child
