摘要
目的探讨宽频声导抗测试对耳硬化症及听骨链畸形在辅助诊断中的应用价值。方法选取就诊于厦门大学附属第一医院耳科门诊的耳硬化症患者40耳,先天性中耳畸形患者10耳,另以正常健康志愿者(70耳)作为正常对照组,所有受试者术前均行宽频声导抗测试,比较各组患者峰压下的声能吸收率。结果峰压下,正常对照组3%(2/70)在1 kHz附近出现一切迹;峰压下,耳硬化症组5%(2/40)在1 kHz附近出现切迹;峰压下,先天性听骨链畸形组70%(7/10)的患者在1kHz附近有一切迹。先天性听骨链畸形组患者在1kHz附近出现切迹的患者比例与耳硬化症组和正常对照组有显著性差异(P<0.05)。结论大部分先天性听骨链畸形患者的宽频声导抗在1 kHz附近会出现一切迹。宽频声导抗测试可作为术前辅助诊断耳硬化症和听骨链畸形的听力学检查方法之一。
OBJECTIVE To investigate the value of wideband acoustic immittance indiagnosing ear sclerosis and bone chain deformity.METHODS Forty patients of otosclerosis and 10 of congenital auditory bone chain deformity were selected,and normal healthy volunteers(70 ears)were taken as the normal control group.Wideband acoustic immittance were tested for all subjects preoperatively to compare acoustic energy absorption at peak pressure in each group.RESULTS Under peak pressure,all traces appeared in near 3%(2/70)of the normal control group at 1 kHz;Under peak pressure,5%(2/40)of the otosclerosis group showed cut at near 1 kHz;70%(7/10)of the congenital auditory bone chain deformity group had all traces around 1 kHz.The proportion of patients with incision near 1 kHz in congenital auditory bone chain deformity was significantly different from that of otosclerosis patients and normal controls(P<0.05).CONCLUSION Most patients with congenital auditory bone chain deformity will appear cut at near 1 kHz in wideband acoustic immittance test.Wideband acoustic immittance test can be used as one of the assisting audiological examination methods for preoperative diagnosis of otosclerosis and auditory bone chain deformity.
