摘要
目的:分析三维平衡训练联合重复经颅磁刺激(Repetitive transcranial magnetic stimulation,rTMS)治疗对脑卒中后偏瘫患者的神经功能、步态及跌倒效能的影响。方法:选取2020年3月至2021年3月本院收治的122例脑卒中后偏瘫患者作为研究对象。采用随机数字表法将所有患者分为对照组和观察组,各61例。对照组采取三维平衡训练方案干预,观察组于对照组基础上增加经颅磁刺激方案干预。对比两组康复疗效、神经功能、下肢运动功能、平衡功能、步态参数及跌倒效能。结果:观察组总治愈率明显高于对照组(P<0.05)。干预后,两组患者美国国立卫生院卒中量表(National institutes of health stroke scale,NIHSS)评分较干预前明显下降,且观察组较对照组改善更低(P<0.05);两组患者Fugl-Meyer下肢运动功能评定量表(Fugl-Meyer assessment for the LE,FMA-LE)评分及Berg平衡量表(Berg balance scale,BBS)评分均较干预前明显上升,且观察组较对照组改善更高(P<0.05);两组步态各参数和跌倒效能各维度均较干预前明显上升,且观察组明显高于对照组(P<0.05)。结论:三维平衡训练联合rTMS于脑卒中后偏瘫患者干预可提高其康复疗效,改善其神经功能、下肢运动功能及平衡功能,提高其行走时效能,增加活动时不跌倒信心,值得推广于临床。
Objective:To analyze the effects of three-dimensional balance training combined with repetitive transcranial magnetic stimulation(rTMS)on neurological function,gait and fall performance in patients with post-stroke hemiplegia.Methods:A total of 122 patients with post-stroke hemiplegia admitted to the hospital from March 2020 to March 2021 were selected as the research subjects.The patients were divided into two groups by random number table method with 61 cases in each group.The control group received three-dimensional balance training,and the observation group received transcranial magnetic stimulation on this basis.Rehabilitation effect,neurological function,lower limb motor function,balance function,gait parameters and fall performance were compared between the two groups.Results:The total cure rate in the observation group was significantly higher than that in the control group(P<0.05).After intervention,the National institutes of health stroke scale(NIHSS)scores of the two groups decreased significantly,while the Fugl-Meyer assessment for the LE(FMA-LE)scores,Berg balance scale(BBS)scores,gait parameters and fall performance scores increased significantly.The improvements in the observation group were better than those in the control group(P<0.05).Conclusion:Three-dimensional balance training combined with rTMS can improve the rehabilitation effect of patients with post-stroke hemiplegia,improve their neurological function,lower limb motor function,balance function and walking performance,and increase their confidence of not falling during activities.
