摘要
目的探讨经皮微创钢板内固定术(MIPPO)结合“骨性窗口滑动技术”治疗肱骨近端骨折的疗效。方法回顾性分析浙江省杭州邦尔医院有限公司2018年2月-2021年2月收治的56例肱骨近端骨折患者的临床资料,均采用MIPPO技术,通过两个软组织窗口,复位骨折并初步获得稳定,再行“骨性窗口滑动技术”,打入内侧柱支撑螺钉,统计手术时间、透视次数及复查X线片骨折愈合情况及愈合时间,采用Neer评分标准评价肩关节功能。结果本组均完成手术治疗,影像学结果显示,术后平均18.3周达到骨性愈合。末次随访时,Neer评分优良率87.5%(49/56),其中,优良率二部分骨折Neer评分为94.7%(18/19),三部分骨折89.3%(25/28),四部分骨折66.7%(6/9)。结论经皮微创钢板内固定技术结合“骨性窗口滑动技术”可以相对扩大工作空间,更准确地置入内侧柱支撑螺钉,实现对关键骨块的准确支撑,也能更好地保护腋神经。
Objective To investigate The efficacy of MIPPO and“bone window sliding technique”for treating proximal humerus fractures.Methods Retrospective analysis of 56 cases of proximal humeral fracture patients admitted in February 2018-2021,using MIPPO technology,through two soft tissue window,reduction fracture and preliminary stability,then“bone window sliding technology”,into the medial column support screw,statistical operation time,fluoroscopy times and review X-ray fracture healing and healing time,using Neer score standard evaluation of shoulder joint function.Results This group completed surgical treatment and imaging results showed mean bone healing after 18.3 weeks.At the last follow-up,the Neer score was 87.5%(49/56),and the Neer score was 94.7%(18/19),89.3%(25/28),and 66.7%(6/9).Conclusion Percutaneous minimally invasive steel plate internal fixation technology combined with“bone window sliding technology”can relatively expand the working space,insert the medial column support screw more accurately,realize the accurate support for the key bone block,and also better protect the axillary nerve.
