摘要
目的探讨皮肤镜对脂溢性角化病和黑素细胞痣的鉴别诊断价值.方法选取2020年12月-2021年12月于江苏省溧阳市中医医院就诊的疑似脂溢性角化病、黑素细胞痣患者87例,依据病理检查结果分为脂溢性角化病(n=41例)和黑素细胞痣(n=46例),分别进行肉眼观察和皮肤镜检查,分析肉眼观察和皮肤镜检查对脂溢性角化病和黑素细胞痣的鉴别诊断价值,分析肉眼检查、皮肤镜检查与病理诊断"金标准"的一致性.结果肉眼观察对脂溢性角化病和黑素细胞痣的阳性预测值、阴性预测值、敏感度、特异度分别为58.14%、63.64%、60.98%、60.87%;皮肤镜检查对脂溢性角化病和黑素细胞痣的阳性预测值、阴性预测值、敏感度、特异度分别为92.50%、93.48%、90.24%、93.48%.肉眼观察的鉴别诊断价值明显低于皮肤镜检查.肉眼观察的鉴别诊断准确率60.92%低于皮肤镜检查的91.95%(P<0.05).皮肤镜检查与病理诊断"金标准"具有一致性(Kappa=0.838,P<0.05).结论皮肤镜对脂溢性角化病和黑素细胞痣有重要的鉴别诊断价值,与病理诊断具有一致性.
Objective To explore the value of dermoscopy in the differential diagnosis of seborrheic keratosis and melanocytic nevus.Methods 87 patients with suspected seborrheic keratosis and melanocytic nevus treated from December 2020 to December 2021 were selected.They were divided into seborrheic keratosis and melanocytic nevus,Visual observation and dermoscopy were performed respectively to analyze the value of visual observation and dermoscopy in the differential diagnosis of seborrheic keratosis and melanocytic nevus.Results The positive predictive value,negative predictive value,sensitivity and specificity of seborrheic keratosis and melanocytic nevus were 58.14%,63.64%,60.98% and 60.87%;the positive predictive value,negative predictive value,sensitivity and specificity of dermoscopy for seborrheic keratosis and melanocytic nevus were 92.50%,93.48%,90.24% and 93.48%.The differential diagnostic value of naked eye observation was significantly lower than that of dermatoscopy.The accuracy of differential diagnosis by naked eye observation was 60.92%,which was lower than that by dermatoscopy 91.95%.The comparison was statistically significant(P<0.05).Dermoscopy was consistent with the"gold standard"of pathological diagnosis(Kappa=0.838,P<0.05).Conclusion Dermoscopy has important differential diagnostic value in seborrheic keratosis and melanocytic nevus,which is consistent with pathological diagnosis.
