摘要
目的分析血液灌流联合连续性血液净化对重症脓毒血症的治疗效果.方法选取北京市石景山医院2020年12月-2022年6月收治的80例重症脓毒血症患者作为研究对象,随机分为两组,每组40例患者,对照组采用常规治疗,观察组在对照组的基础上使用血液灌流联合连续性血液净化治疗.比较两组患者的炎症因子水平、血乳酸水平以及各项生命体征.结果观察组和对照组TNF-α、CRP、PCT治疗后均低于治疗前,且观察组炎症因子水平均优于对照组,差异有统计学意义(P<0.05).观察组和对照组PT、APTT治疗后均低于治疗前,而Fbg、PLT在治疗后高于治疗前,且观察组凝血功能水平变化均优于对照组,差异有统计学意义(P<0.05).观察组和对照组呼吸频率、心率在治疗后低于治疗前,脉压高于治疗前.且观察组各项生命体征均优于对照组,差异有统计学意义(P<0.05).结论经过血液灌流联合连续性血液净化治疗后的重症脓毒血症患者,其临床疗效显著,能有效改善患者的各项生命体征,减轻炎症反应,具有较高的临床应用价值.
Objective To analyze the treatment of severe sepsis by hemoperfusion combined with continuous blood purification.Methods A total of 80 patients with severe sepsis who were admitted to our hospital from Decemher 2020 to June 2022 were selected as the research objects,and randomly divided into two groups with 40 patients in each group.The control group received conventional treatment,and the ohservation group on the basis of the control group,blood perfusion combined with continuous blood purification was used.The levels of inflammatory factors,blood lactate and various vital signs were compared between the two groups.Results The levels of TNF-α,CRP,and PCT in the observation group and the control group after treatment were lower than those before treatment,and the levels of inflammatory factors in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).After treatment,PT and APTT in the observation group and control group were lower than those before treatment,while Fbg and PLT were higher after treatment than before treatment,and the changes of blood coagulation function in the observation group were better than those in the control group,with statistical significance(P<O.05).The respiratory rate and heart rate of the observation group and the control group after treatment were lower than those before treatment,and the pulse pressure was higher than that before treatment.The vital signs of the observation group were better than those of the control group,and the difference was statistically significant(P<0.05).Conc lus ion After hemoperfusion combined with continuous blood purification in severe sepsis patients,the clinical efficacy is significant,and it can effectively improve the vital signs of patients and reduce inflammatory response,which has high clinical application value.
