摘要
目的分析静脉配置中心常见抗生素使用问题及干预措施.方法选取2020年1月-2021年10月无锡市人民医院静脉配置中心120张抗生素处方,分析处方中抗生素联合用药、用量和用法等情况,按照有关规定、药品管理使用说明书、药理学知识等判断使用不合理处方,明确不合理应用因素,并提出干预对策.结果120张抗生素处方中,总计48张不合理处方,占比为40.00%;具体因素:不良反应增加占比3.33%,重复用药占比4.17%,无指征用药占比3.33%,溶媒不当占比2.50%,药物拮抗占比6.67%,超剂量用药占比12.50%,用药时间不合理占比7.50%.结论临床上抗生素使用不合理发生率较高,以超剂量用药、用药时间不合理等为主.导致这一现象的因素较多.通过实施针对性干预对策,可尽量保证抗生素药物应用的科学性及合理性,从而为临床医生治疗方案的制定提供参考.
Objective To analyze the common problems in the use of antibiotics in intravenous admixture service centers and the intervention measures.Methods Select 120 antibiotic prescriptions from January 2020 to October 2021 in the intravenous dispensing center of our hospital,analyze the situation of combined use,dosage and usage of antibiotics in the prescriptions,judge the use of unreasonable prescriptions according to relevant regulations,drug management instructions,pharmacological knowledge,etc.,identify the factors of unreasonable use,and propose intervention countermeasures.Results Among 120 antibiotic prescriptions,48 prescriptions were unreasonable,accounting for 40.00%;specific factors:adverse reactions accounted for 3.33%,repeated drug use accounted for 4.17%,no indication drug use accounted for 3.33%,improper solvent accounted for 2.50%,drug antagonism accounted for 6.67%,overdose drug use accounted for 12.50%,and irrational drug use accounted for 7.50%.Conclusion The incidence of irrational use of antibiotics in clinical practice is high,mainly including overdose and unreasonable time of use.There are many factors leading to this phenomenon.Through the implementation of targeted intervention strategies,the scientific and rational use of antibiotics can be ensured as far as possible,so as to provide reference for clinicians to formulate treatment plans.
