摘要
目的探讨口腔颌面部恶性肿瘤患者术后并发深静脉血栓的危险因素。方法回顾性研究收住的144例口腔颌面部恶性肿瘤患者的临床资料,分为非血栓组108例,血栓组36例。对深静脉血栓发生率进行单因素分析和logistic回归分析,对P<0.05的危险因素进一步进行受试者操作特征曲线分析。结果深静脉血栓发生率为25%,肺栓塞发生率为4.2%。单因素分析中手术时长与术后卧床时长差异具有统计学意义(P<0.05),多因素分析中仅有手术时长与深静脉血栓显著相关(P<0.05)。受试者操作特征曲线分析表明手术时长的截止值为6.75 h,敏感度51.7%,特异度为72.2%,模型曲线下面积为0.641。结论本研究中,手术时长是口腔颌面恶性肿瘤患者术后并发深静脉血栓的独立危险因素;手术时长超过6 h,深静脉血栓发生率显著增高。临床上应根据这一特征积极实施深静脉血栓的预防与诊治措施,降低深静脉血栓的发生。
Objective To explore the risk factors of postoperative deep vein thrombosis in patients with oral and maxillofacial malignancies tumors.Methods 144 patients with oral and maxillofacial malignant tumors were retrospectively investigated.They were divided into non-thrombotic group(108 cases)and thrombotic group(36 cases).The incidence of deep vein thrombosis was analyzed by Univariate analysis and Logistic regression analysis,and the risk factors(P<0.05)were further analyzed by subject operation characteristic curve.Results The incidence of deep vein thrombosis was 25%,and the incidence of pulmonary embolism was 4.2%.In the univariate analysis,the length of operation(P<0.05)and the length of postoperative bed rest(P<0.05)were statistically significant.In the multivariate analysis,only the length of operation(P<0.05)was significantly related to the occurrence of deep vein thrombosis.The analysis of the operation characteristic curve of the subjects showed that the cut-off value of the operation time was 6.75 hours,the sensitivity was 51.7%,the specificity was 72.2%,and the area under the model curve was 0.641.Conclusion In this study,the length of operation is an independent risk factor for deep vein thrombosis in patients with oral and maxillofacial malignancies.The measures for the prevention,diagnosis and treatment of deep vein thrombosis should be actively implemented to reduce the occurrence of deep vein thrombosis.
