摘要
目的分析小儿心肌致密化不全(NVM)的临床特点及超声心动图特征,探讨超声心动图在小儿NVM诊断中的临床价值。方法30例小儿NVM患儿,均采用超声心动图检查,分析患儿的诊断结果、临床特点、超声心动图特征。结果患儿中孤立型20例,非孤立型10例。30例患儿经检查显示左室型17例,右室型8例,双室型5例。超声心动图显示左心扩大17例,右心扩大7例,全心扩大5例,房室正常1例。病变位置主要包括心尖部合并中断侧壁位置,包括下壁心肌12例,心尖部心肌8例,基底段心肌3例以及室壁心肌7例。随访3例患儿死于心力衰竭(心衰)或多器官功能衰竭,死亡率为10.00%。结论先天性心脏病患儿发生NVM的风险较高,容易引发患儿心衰,其临床症状表现与NVM的范围关系密切,孤立型患儿早期阶段症状表现具有隐匿性,对患儿使用超声心动图进行诊断,对于评估患儿心脏功能,改善患儿预后具有重要意义。
Objective To analyze the clinical characteristics and echocardiographic features of noncompaction of the ventricular myocardium(NVM)in children,and discuss the clinical value of echocardiography in the diagnosis of NVM in children.Methods 30 children with NVM were examined by echocardiography,and the diagnostic results,clinical characteristics,and echocardiographic features of the children were analyzed.Results There were 20 cases of isolated type and 10 cases of non-isolated type among the children.Echocardiography showed 17 cases of left ventricular type,8 cases of right ventricular type and 5 cases of double ventricular type.Echocardiography showed left heart enlargement in 17 cases,right heart enlargement in 7 cases,whole heart enlargement in 5 cases,and normal atrium and ventricle in 1 case.The location of the lesions mainly included the apical region combined with the interrupted lateral wall location,including the inferior wall myocardium in 12 cases,the apical myocardium in 8 cases,the basal segment myocardium in 3 cases and the ventricular wall myocardium in 7 cases.3 children died of heart failure or multiorgan failure at follow-up,with a mortality rate of 10.00%.Conclusion The risk of NVM in children with congenital heart disease is high,and it is easy to cause heart failure in children.The clinical symptom expression is closely related to the range of NVM,and the symptom expression in the early stage of children with isolated type is insidious.The use of echocardiography in the diagnosis of children is of great significance to evaluate the cardiac function and improve the prognosis of children.
