摘要
目的:研究单纯椎板减压术(simple vertebral canal decompression,SVCD)治疗退行性腰椎管狭窄症(degenerative lumbar spinal stenosis,DLSS)的效果。方法:根据随机数字表法将2021年6月-2022年5月于江苏大学附属人民医院接受治疗的98例DLSS患者分为对照组与研究组,各49例。对照组采用腰椎后路减压内固定融合术(posterior lumbar intervertebral fusion,PLIF)治疗,研究组采用SVCD治疗。对比两组手术情况,治疗效果,手术前后的脊柱矢状面平衡参数、脊髓功能、疼痛情况及腰椎功能。结果:两组总有效率比较,差异无统计学意义(P>0.05);研究组手术时间、术中出血量与术后卧床时间均明显优于对照组(P<0.05);两组术后6个月腰椎前凸角、腰椎后凸角、L4腰椎倾斜率均较术前明显改善,且研究组较对照组改善更明显(P<0.05);两组术后6个月腓浅神经、腓深神经的传导速度明显高于术前,且研究组明显高于对照组,腓浅神经、腓深神经的潜伏期水平明显低于术前,且研究组明显低于对照组(P<0.05);两组术后6个月的视觉模拟评分法(visual analogue scale,VAS)评分、日本骨科协会评估治疗分数(Japanese Orthopaedic Association score,JOA)、Oswestry功能障碍指数(Oswestry disability index,ODI)均优于术前,但两组VAS、ODI、JOA评分比较,差异均无统计学意义(P>0.05)。结论:SVCD治疗DLSS可明显改善患者手术情况,恢复脊椎矢状面平衡,减少脊髓功能损伤,并减轻疼痛,促进腰椎功能恢复,具有较好的治疗效果。
Objective:To investigate the effect of simple vertebral canal decompression(SVCD)in the treatment of degenerative lumbar spinal stenosis(DLSS).Method:According to the random number table method,98 DLSS patients who came to Affiliated People’s Hospital of Jiangsu University for treatment from June 2021 to May 2022 were divided into the control group and the research group,with 49 cases in each group.The control group was treated with posterior lumbar interbody fusion(PLIF),and the research group was treated with SVCD.The operation situation,treatment effect,before and after operation of spine sagittal balance parameters,spinal cord function,pain situation and lumbar function were compared between the two groups.Result:There was no significant difference in the total effective rate between the two groups(P>0.05).The operation time,intraoperative bleeding volume and postoperative bed rest time of the research group were significantly better than those of the control group(P<0.05).6 months after operation,the lumbar lordosis,lumbar kyphosis and L4 lumbar inclination rate in both groups were significantly improved,and the improvement in the research group were more obvious than that in the control group(P<0.05).6 months after operation,the conduction velocities of superficial peroneal nerve and deep peroneal nerve in the two groups were significantly higher than those before operation,and the latency levels of superficial peroneal nerve and deep peroneal nerve in the research group were significantly lower than those in the control group(P<0.05).The visual analogue scale(VAS)scores,Japanese Orthopaedic Association scores(JOA)and Oswestry disability index(ODI)of the two groups after 6 months operation were better than those before operation,but there were no significant differences in the scores of VAS,ODI and JOA between the two groups(P>0.05).Conclusion:SVCD in the treatment of DLSS can obviously improve the patients’operation situation,restore the spine sagittal balance,reduce spinal cord functional injury,relie
