摘要
目的:研究超声心动图联合Tei指数对脓毒症预后评估价值及对心肌损伤的诊断价值。方法:选取2019年3月-2022年3月佳木斯市中心医院收治的脓毒症患者118例,根据心肌损伤标志物、心功能标志物和体征异常情况将其分为损伤组和非损伤组,对比两组确诊时的超声心动图水平和Tei指数,将损伤组患者根据28 d随访结果分为存活组和死亡组,对比其确诊时超声心动图和Tei指数。结果:损伤组左心室射血分数(LVEF)、二尖瓣环口舒张早期流速/心房收缩期最大流速(E/A)均低于非损伤组,左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、Tei指数均高于非损伤组(P<0.05),两组右心室内径(RVD)差异无统计学意义(P>0.05);存活组LVEF高于死亡组,LVESD、LVEDD、Tei指数均低于死亡组(P<0.05),两组RVD、E/A差异均无统计学意义(P>0.05);ROC分析结果提示,超声心动图联合Tei指数对于预后结果预测价值高于单一预测。结论:超声心动图数据和Tei指数的异常情况对于判断脓毒症患者是否出现心肌损伤有一定意义,且二者联合对于判断患者预后结果有较高价值,值得推广。
Objective:To study the prognostic value of echocardiography combined with Tei index in sepsis and the diagnostic value of myocardial injury.Method:A total of 118 patients with sepsis admitted to Jiamusi Central Hospital from March 2019 to March 2022 were selected.They were divided into injury group and non-injury group according to the abnormalities of myocardial injury markers,cardiac function markers and physical signs,the echocardiographic levels and Tei indexes of the two groups at the time of diagnosis were compared between the two groups.Patients in the injury group were divided into a survival group and a death group based on 28 d follow-up,and the echocardiographic levels and Tei indexes at the time of diagnosis were compared.Result:LVEF and E/A in the injury group were lower than those in the non-injury group,and LVESD,LVEDD,and Tei indexes were higher than those in the non-injury group(P<0.05).There was no significant difference in RVD between the two groups(P>0.05).LVEF in the survivor group was higher than that in the death group,and LVESD,LVEDD,and Tei indexes were lower than those in the death group(P<0.05),and there were no significant differences in RVD and E/A between the two groups(P>0.05).The results of the ROC analysis suggested that the predictive value of echocardiography combined with Tei index for prognosis was higher than that of single prediction.Conclusion:The abnormalities of echocardiographic data and Tei index are meaningful in determining whether myocardial injury is present in patients with sepsis,and the combination of the two has high value in determining the prognostic outcome of patients,which is worth promoting.
