摘要
目的探讨外侧壁的完整性对防旋股骨近端髓内钉(PFNA)治疗股骨转子间骨折的临床疗效。方法回顾性分析我院2017年1月至2020年6月30日所收治的股骨转子间骨折病例80例。均采用PFNA治疗。根据术前X片及三维CT所示股骨近端外侧壁的情况将所有患者分成外侧壁完整组和外侧壁不完整组。两组患者年龄、营养状况(白蛋白)、骨密度(QCT)值、合并症等一般资料的差异无统计学意义(P>0.05),比较两组患者术中出血量、手术时间、术后住院天数、随访术后部分负重时间、负重时间、骨折愈合时间。采用Harris髋关节功能评分评估患者术后1年髋关节功能恢复情况。结果外侧壁完整组在术中出血量、手术时间、术后住院天数、术后部分负重时间、负重时间、骨折愈合时间均小于外侧壁不完整组,差异有统计学意义(P<0.05)。外侧壁完整组髋关节术后1年功能评分大于不完整组,髋关节功能优于不完整组(t=-2.25,P=0.03)。结论外侧壁的完整性影响股骨转子间骨折术后内固定稳定性,影响骨折愈合及髋关节功能。
Objective To investigate clinical effect of PFNA on treatment of femoral intertrochanteric fractures by integrity of lateral wall.Methods From January 2017 to June 30,2020,retrospective analysis of 80 cases of femoral intertrochanteric fractures admitted in our hospital.All were treated with PFNA.According to preoperative X-ray and three-dimensional CT of lateral wall of proximal femur,all cases were divided into a complete lateral wall group and an incomplete lateral lateral wall group.P values of general information,such as age,nutritional status(albumin),Bone density(QCT),and comorbidities in two groups,and difference was not statistically significant(P>0.05).Intraoperative blood loss,operation time,postoperative hospitalization days,follow-up postoperative partial weight-bearing time,weight-bearing time,and fracture healing time were compared.Harris Hip Function Score was used to evaluate recovery of hip joint function one year after operation.Results Intraoperative blood loss,operation time,postoperative days of hospitalization,postoperative partial weight bearing time,weight bearing time,and fracture healing time in the complete lateral wall group were less than those in the incomplete lateral wall group.Differences were statistically significant(P<0.05).Functional score of hip joint in the intact lateral wall group was greater than that in the incomplete group,and hip joint function was better than that in the incomplete group(t=-2.25,P=0.03).Conclusion Integrity of lateral wall would affect stability of internal fixation for femoral intertrochanteric fractures,as well as fracture healing and hip joint function.
