Th17/Treg、TGF-β1、IL-17与急性脑卒中的病情严重程度以及预后的相关性研究 被引量:5

摘要 目的:探讨辅助性T细胞(Th17)/调节性T细胞(Treg)、转化生长因子β1(TGF-β1)、白细胞介素17(IL-17)与急性脑卒中的病情严重程度以及预后的相关性。方法:选取2019年1月至2022年1月来川北医学院附属医院治疗的急性脑卒中患者147例作为观察组,选择同期100例门诊体检健康者作为对照组。根据美国国立卫生研究院卒中量表(NIHSS)评分将观察组患者分为轻度组、中度组、重度组,出院后3个月根据改良Rakin量表评分(mRS)分为预后良好组和预后不良组。检测并比较各组研究对象外周血Th17/Treg、TGF-β1和IL-17水平。结果:观察组和对照组的Th17/Treg、TGF-β1和IL-17比较,差异存在统计学意义(P<0.05),与对照组相比,观察组的Th17/Treg升高,TGF-β1和IL-17水平显著降低。轻度组、中度组、重度组血清Th17/Treg、TGF-β1和IL-17比较差异存在统计学意义(P<0.05),其中血清Th17/Treg水平逐渐升高,TGF-β1、IL-17则逐渐降低;预后良好组与预后不良组的血清Th17/Treg、TGF-β1和IL-17比较差异也存在统计学意义(P<0.05),预后不良组血清Th17/Treg明显高于预后良好组,TGF-β1和IL-17明显低于预后良好组。Pearson相关性分析显示,血清Th17/Treg与INHSS、mRS评分呈明显正相关,TGF-β1和IL-17与NIHSS、mRS评分呈明显负相关(P<0.05)。结论:Th17/Treg、TGF-β1和IL-17与急性脑卒中患者病情严重程度和临床预后相关,可能是通过介导相应的炎症反应来发挥作用,可以为急性脑卒中患者病情严重程度和预后情况的临床评价提供重要参考。 Objective:To investigate the correlation between T helper cell(Th17)/regulatory T cell(Treg),transforming growth factorβ1(TGF-β1),interleukin 17(IL-17)and the severity and prognosis of acute stroke.Methods:A total of 147 patients with acute stroke admitted to Affiliated Hospital of North Sichuan Medical College between January 2019 and January 2022 were included as the study group.A contemporary cohort of 100 healthy subjects were included as the control group.Patients in the study group were divided into mild group,moderate group and severe group according to National Institutes of Health Stroke Scale(NIHSS)score.At 3 months after discharge,the patients in the study group were divided into good prognosis group and poor prognosis group according to the modified Rakin scale(mRS)score.The levels of peripheral blood Th17/Treg,TGF-β1 and IL-17 were determined and compared between the groups.Results:There were statistically significant differences in the levels of Th17/Treg,TGF-β1 and IL-17 between the study group and control group(P<0.05).Compared with the control group,the Th17/Treg level increased in the study group,whereas the levels of TGF-β1 and IL-17 significantly decreased.There were statistically significant differences in the serum levels of Th17/Treg,TGF-β1 and IL-17 among the mild,moderate and severe groups(all P<0.05),the serum level of Th17/Treg increased gradually,while the serum levels of TGF-β1 and IL-17 decreased gradually(all P<0.05).There were statistically significant differences in the serum levels of Th17/Treg,TGF-β1 and IL-17 between the good prognosis group and the poor prognosis group(all P<0.05),the serum Th17/Treg level in the poor prognosis group was significantly higher than that in the good prognosis group,whereas the serum levels of TGF-β1 and IL-17 were significantly lower than those in the good prognosis group.Pearson correlation analysis showed that serum Th17/Treg was significantly positively correlated with NIHSS and mRS scores,while TGF-β1and IL-17 were significantly neg
