摘要
目的分析输尿管镜下钬激光碎石术后并发输尿管狭窄的影响因素。方法回顾性分析2019年1月至2021年12月上饶东信第五医院收治的100例输尿管镜下行钬激光碎石术输尿管结石患者的临床资料。根据术后是否并发输尿管狭窄分为未发生组(n=87)和发生组(n=13),比较两组可能的影响因素,分析输尿管镜下钬激光碎石术后并发输尿管狭窄的影响因素。结果患者病程(β=0.963,OR=2.620,95%CI=1.945~3.529,P<0.001)、结石直径(β=1.247,OR=3.480,95%CI=2.827~4.283,P<0.001)、息肉包绕结石情况(β=2.248,OR=9.469,95%CI=0.595~150.734,P=0.013)、结石嵌顿情况(β=1.692,OR=5.430,95%CI=0.660~44.657,P=0.015)、输尿管镜类型(β=1.216,OR=3.374,95%CI=1.119~10.170,P<0.001)是输尿管镜下钬激光碎石术后并发输尿管狭窄的影响因素(P<0.05)。结论对于病程较长、结石嵌顿、结石直径较大、息肉包绕结石的患者来说,并发输尿管狭窄的风险较大,此外输尿管镜较粗也是并发输尿管狭窄的危险因素。
Objective To analyze the influencing factors of ureteral stricture after ureteroscopic holmium laser lithotripsy.Methods The clinical data of 100 patients with ureteral calculi undergoing ureteroscopic holmium laser lithotripsy admitted to Shangrao Dongxin Fifth Hospital from January 2019 to December 2021 were retrospectively analyzed.According to whether ureteral stenosis was complicated after operation,the patients were divided into the non-occurrence group(n=87)and the occurrence group(n=13).The possible influencing factors of the two groups were compared,and the influencing factors of ureteral stenosis complicated after ureteroscopic holmium laser lithotripsy were analyzed.Results The course of disease(β=0.963,OR=2.620,95%CI=1.945-3.529,P<0.001),stone diameter(β=1.247,OR=3.480,95%CI=2.827-4.283,P<0.001),polyp surrounding calculus(β=2.248,OR=9.469,95%CI=0.595-150.734,P=0.013),stone entrapment(β=1.692,OR=5.430,95%CI=0.660-44.657,P=0.015),type of ureteroscopy(β=1.216,OR=3.374,95%CI=1.119-10.170,P<0.001)were influential factors for ureteral stricture after ureteroscopic holmium laser lithotripsy(P<0.05).Conclusion For patients with long course of disease,stone incarceration,large stone diameter and polyp around the stone,the risk of complicated ureteral stenosis is greater.In addition,thicker ureteroscope is also a risk factor for complicated ureteral stenosis.
