摘要
近年来,海水浸泡伤发生率逐渐增高,海水环境特殊,患者负伤落水后除自身伤势外,还可能发生软组织坏死、感染概率增加、炎症反应加重、低体温、水电酸碱平衡紊乱、微循环障碍等。正确的伤口冲洗可有效降低海水浸泡伤伤口的感染风险,使患者及时脱离海水环境,延缓伤情进一步发展,对改善患者预后具有重要意义。目前,针对海水浸泡伤伤口最佳的冲洗液种类、压力、体积和温度尚无定论,缺乏动物实验和高质量的临床研究来进行相关验证。本文对冲洗压力、冲洗液种类、冲洗水流方向、冲洗时间(液体用量)等影响伤口冲洗效果的因素进行探讨,以期为进一步开展相关研究提供参考。
In recent years,the incidence of seawater-immersed wounds is increasing.The seawater environment is special and thus the wounded may additionally suffer from soft tissue necrosis,increased probability of infection,aggravated inflammatory response,hypothermia,acid-base disorders,and microcirculatory disorders after seawater drowning.Correct management of the wound like irrigation can effectively reduce the risk of infection,timely change the seawater environment at the wound and slow down the wound progression,all of which are of great importance to improve the outcomes of the wounded.At present,the optimal type,pressure,volume and temperature of the irrigation fluid for seawater-immersed wounds have not yet been confirmed,and there is a lack of animal studies and high-quality clinical studies for verification.Therefore,the affecting factors including the washing fluid types and the flow direction,duration as well as the volume at irrigation are discussed in the review.
