摘要
目的观察壮药溻肿熏洗剂联合尿激酶溶栓在自体动静脉内瘘急性血栓中的应用效果。方法选择92例自体动静脉内瘘急性血栓患者作为研究对象,根据随机信封抽签原则分为治疗组和对照组,各46例。对照组予尿激酶溶栓治疗,治疗组在对照组治疗基础上加壮药溻肿熏洗剂治疗,2组均3 d为1个疗程,治疗1个疗程后比较疗效,比较2组治疗前后凝血功能指标血浆凝血酶原时间(PT)、纤维蛋白原(FIB)、活化部分凝血活酶时间(APTT)、D-二聚体变化,统计比较2组治疗过程中出现的不良反应。结果治疗后,2组PT、APTT均较本组治疗前缩短(P<0.05),FIB、D-二聚体水平均较本组治疗前降低(P<0.05),且治疗组PT、APTT短于对照组(P<0.05),FIB、D-二聚体水平低于对照组(P<0.05)。治疗组总有效率97.8%(45/46),对照组总有效率84.8%(39/46),治疗组疗效优于对照组(P<0.05)。治疗期间治疗组并发症发生率为10.9%(5/46),对照组为28.3%(13/46),治疗组并发症发生率低于对照组(P<0.05)。结论壮药溻肿熏洗剂联合尿激酶溶栓治疗自体动静脉内瘘急性血栓能改善患者凝血功能,提高治疗效果,且能减少并发症的发生。
Objective To observe the effect of Zhuang Medicine Tazhong fumigation washing form combined with urokinase thrombolysis for autogenous arteriovenous fistula(AVF)with acute-phase thrombosis.Methods Totally 92 autogenous AVF patients with acute-phase thrombosis were randomly assigned at 1:1 ratio to receive either urokinase thrombolysis(control group)or Zhuang Medicine Tazhong fumigation washing+urokinase thrombolysis(treatment group)for 1 courses(1 course for 3 days).The coagulation function indexes(plasma prothrombin time[PT],fibrinogen[FIB],activated partial thromboplastin time[APTT],D-dimer[D-D]),curative effect,and adverse reactions were included as comparators.Results The two groups had shortened PT and APTT and decreased FIB and D-D after treatment(all P<0.05),the treatment group was superior to the control group for PT,APTT,FIB and D-D(P<0.05).The overall effective rate in treatment group was markedly better than that in control group(97.8%[45/46]vs 84.8%[39/46],[P<0.05],respectively).The complications was 10.9%(5/46)in treatment group,significantly lower than that of 28.3%(13/46)in control group(P<0.05).Conclusion For autogenous AVF patients with acute thrombosis,Zhuang Medicine Tazhong fumigation washing combined with urokinase thrombolysis is capable improving coagulation function,with effective therapeutic effects and reduced complications.
