摘要
从肾论治双心疾病的具体治则治法如下:①心肾阳虚证治宜温补肾阳,振奋心阳,可兼健脾利水;方以补坎益离丹加减,下肢浮肿明显者宜真武汤合桂枝甘草汤加减。②心肾阴虚证治宜滋阴清火,养心和络;若阴液不足,心失所养,心动悸脉结代者,方选炙甘草汤加减;若阴虚火旺,虚热内扰,心烦不得眠者方选黄连阿胶汤合酸枣仁汤;若腰膝酸软,头发枯槁,心悸不宁,方选六味地黄汤加减;若虚阳上浮,肝风内动者加用龙骨、牡蛎、灵磁石、珍珠母等重镇潜阳药物。③寒凝心脉证治宜温阳散寒,温通心阳;若胸闷气短,手足不温,遇冷加重者,方选枳实薤白桂枝汤合当归四逆汤;若寒凝心脉重症遇冷加重,心痛彻背,背痛彻心,方选乌头赤石脂丸加减。④水饮凌心证治宜温补肾阳,振奋心阳,健脾利水;若心悸眩晕症状明显,恶心欲吐,口吐涎液,方选苓桂术甘汤合半夏白术天麻汤加干姜、附子;若下肢浮肿,形寒肢冷,小便清而少,方选真武汤加减。
The specific treatment principles and methods of treating Heart disease in double aspects from perspective of Kidney are as follows:①The treatment of deficiency of Heart and Kidney Yang should warm and replenish the Kidney Yang,invigorate the Heart Yang,and strengthen the Spleen to remove the pathological Water.Modified Bukan Yili Dan can be used as the prescription.If the lower limb edema is obvious,modified Zhenwu Decoction and Guizhi Gancao Decoction should be used.②The treatment of Heart and Kidney Yin deficiency,we should nourish Yin to clear Fire,to nourish Heart and harmonize collaterals,If the Yin fluid is insufficient and the Heart cannot be nourished,the heart palpitates with slow or irregular pulse,modified Zhigancao Decoction can be used.If Yin deficiency is serious with strong pathological Fire and internal disturbance,which cause people upset and sleepless,Huanglian Ejiao Decoction and Suanzaoren Decoction can be used.If the patient suffers from sore waist and knees,withered hair,with the restless heart palpitates,modified Liuwei Dihuang Decoction can be used.If the asthenic Yang rises and the Liver Wind moves inwards,Longgu(ossa draconis),Muli(oyster),Lingcishi(magnetite),Zhenzhumu(concha margaritiferallsta)and other important Yang-hiding drugs can be added in the prescription.③In treatment of Cold coagulation in Heart,we should warm Yang to disperse Cold and warm Heart Yang.If the chest is stuffy and short of breath,with cold hands and feet and getting worse with coldness,Zhishi Xiebai Guizhi Decoction and Danggui Sini Decoction can be used in combination.For severe cases with aggravated cold,deep heartache to the back,or the deep backache to the Heart,modified Wutou Chishizhi Pill can be used.④In treatment of the syndrome of Water injuring Heart,we should warm and replenish the Kidney Yang,to invigorate the Heart yang,strengthen the Spleen to remove Water.If the symptoms of palpitation and dizziness are obvious,with nausea and vomiting,saliva spit at the mouth,the prescription should be L
