摘要
目的:基于真实世界稳定性冠心病(SCAD)病人的临床数据结合数据挖掘探讨SCAD病人的中医证候特点及中药用药规律。方法:收集2017年1月—2021年5月在中日友好医院中西医结合心脏内科门诊及住院的SCAD病人的中医证候信息及中药处方。建立数据库,使用Microsoft Excel 2016、SPSS Modeler 18.0、IBM SPSS Statistics 25.0、Cytoscape 3.7.2统计学软件进行数据挖掘。结果:共纳入住院病人690例,中医证型以气虚血瘀常见,门诊处方132首,涉及257味中药,高频药物包括黄芪、川芎、柴胡等30味,得出高频药物的性味及归经、功效分类,发掘关联规则常用药对及4个聚类方。结论:SCAD病人临床多见气虚、血瘀、痰浊等证候要素,证型以两证或三证要素夹杂,治疗以益气、活血、宣痹化痰为主,善用经方、效方。
Objective: To explore the characteristics of traditional Chinese medicine syndromes and the rule of traditional Chinese medicine in patients with stable coronary artery disease.Methods: The traditional Chinese medicine syndromes and herbal prescriptions were collected from January 2017 to May 2021.A database was established and data mining was performed using Microsoft Excel 2016,SPSS Modeler 18.0,IBM SPSS Statistics 25.0,and Cytoscape 3.7.2 statistical software.Results: A total of 690 inpatients were included.Qi deficiency and blood stasis were the most common syndrome type of traditional Chinese medicine.There were 132 outpatient prescriptions involved 257 traditional Chinese medicines, high frequency drugs were Astragalus, Chuanxiong, Bupleurum and 30 other flavors.The character, taste, channel distribution and effect of high-frequency drugs were obtained, and common drug pairs of association rules and four clustering prescriptions were found.Conclusion: The mostly common clinical symptoms of stable coronary artery disease were Qi deficiency, blood stasis, and phlegm, and the traditional Chinese medicine syndromes were mixed with two or three syndrome elements.The treatment was based on benefiting Qi, invigorating blood, promoting paralysis and resolving phlegm, with the meridian formula and effective formula.
