摘要
目的:探索郑州市居民对分级诊疗的认知程度,分析个体特征、社会特征和医疗特征对居民分级诊疗认知程度的影响,为提高居民分级诊疗认知、推动分级诊疗建设提供参考。方法:在郑州市抽取712名居民参与问卷调查。对数据进行描述性统计、探索性因子分析和多重线性回归分析。结果:郑州市居民的分级诊疗了解程度与认可程度转换后的因子值分别为36.40和59.44;多重线性回归结果显示,居民的分级诊疗认知程度主要受到社会特征和医疗特征的影响,其中社会特征主要通过教育程度和社区宣传影响居民对分级诊疗的了解程度(P<0.05),医疗特征主要通过居民对基层医疗服务和医疗保险报销政策的感知影响其对分级诊疗的了解和认可程度(P<0.05)。结论:郑州市居民对分级诊疗的总体认知程度不高,社会特征和医疗特征会对居民的分级诊疗认知程度产生显著影响,应合理采取加大宣传力度、提升基层医疗服务水平、拉大医疗保险的报销比例差距等举措,有针对性地提高居民的分级诊疗认知程度,从而为分级诊疗的实现奠定广泛的社会基础。
Objective: To explore the cognition of hierarchical medical system of residents in Zhengzhou, and to analyze the influencing factors from the aspects of individual characteristics, social characteristics and medical characteristics, so as to provide references for improving residents’ cognition of hierarchical medical system and promoting the construction of hierarchical medical system. Methods: A total of 712 residents in Zhengzhou were selected to participate in a questionnaire survey. Data were analyzed by descriptive statistics, exploratory factor analysis and multiple linear regression. Results: The transformed factor values of Zhengzhou residents’ understanding and recognition of hierarchical medical system were 36.40 and 59.44 respectively. The results of multiple linear regression showed that residents’ cognition of hierarchical medical system was mainly affected by social characteristics and medical characteristics. Among them, social characteristics mainly affected residents’ understanding of hierarchical medical system through education level and community publicity(P<0.05). Medical characteristics mainly affected residents’ understanding and recognition of hierarchical medical system through residents’ perception of grassroots medical services and medical insurance reimbursement policies(P<0.05). Conclusion: Zhengzhou residents’ overall cognition of hierarchical medical system is not high, and social characteristics and medical characteristics have a significant impact on residents’ cognition of hierarchical medical system. Therefore, measures such as increasing publicity efforts, improving the level of grassroots medical services, and widening the gap in the reimbursement ratio of medical insurance should be taken reasonably to improve residents’ cognition of hierarchical medical system, so as to lay a broad social foundation for the realization of hierarchical medical system.
