中性粒细胞/淋巴细胞比值、血同型半胱氨酸和超敏C反应蛋白与急性脑梗死患者脑动脉狭窄严重程度的相关性研究 被引量:5

摘要 目的 探讨中性粒细胞/淋巴细胞比值(NLR)、血同型半胱氨酸(Hcy)和超敏C反应蛋白(hs-CRP)与急性脑梗死患者脑动脉狭窄严重程度的相关性。方法 选取我院就诊的依据头颈CTA/DSA影像明确诊断伴脑动脉狭窄的急性脑梗死患者(脑梗死组)220例及同期因头痛头晕住院者(对照组)50例,比较各组基本资料信息、Hcy、hs-CRP、NLR等的差异。结果 急性脑梗死组血Hcy、hs-CRP和NLR均明显高于对照组(均P<0.05)。将急性脑梗死组患者按脑动脉狭窄严重程度进一步分级后显示,脑动脉狭窄严重程度越高,Hcy、hs-CRP和NLR水平越高,差异有统计学意义(均P<0.05)。Spearman相关性分析显示,脑动脉狭窄严重程度与Hcy、hs-CRP和NLR呈显著正相关。多元Logistic回归分析显示,在控制各项危险因素后,Hcy、hs-CRP、NLR与脑动脉狭窄严重程度仍密切相关,是脑动脉狭窄的独立危险因素(均P<0.05)。结论 NLR、Hcy和hs-CRP与急性脑梗死有明显的相关性,NLR、Hcy和hs-CRP的增高是脑动脉狭窄的独立危险因素。 Objective To explore the correlation between neutrophil-to-lymphocyte ratio(NLR)、Hcy、high-sensitivity c-reactive(hs-CRP) and theseverity of cerebral artery stenosis(CAS) in acute cerebral infarction. Methods A total of 220 cases of acute cerebral infarction with CAS diagnosed by CTA/DSA who underwent treatment in our hospital(cerebral infarction group) and 50 patients hospitalized for physical examination due to headache or dizziness in the same period(control group). Then compare the baseline data, Hcy, hs-CRP and NLR of each group. Results The level of Hcy、hs-CRP and NLR in cerebral infarction group were significantly higher than those in control group(all P<0.05). Further classification of patients in the acute cerebral infarction group according to the severity of cerebral artery stenosis showed that the higher the severity of cerebral artery stenosis was, the higher the levels of Hcy, HS-CRP and NLR were, and the differences were statistically significant(all P<0.05). Spearman correlation analysis revealed that Hcy, hs-CRP and NLR were positively correlated with the severity of CAS. Multivariate Logistic regression analysis showed that there were positive correlations between the severity of CAS and Hcy, hs-CRP and NLR, Hcy, hs-CRP and NLR were independent risk factors for the severity of CAS(all P<0.05).Conclusions There are positive correlations between NLR、Hcy、hs-CRP and the severity of CAS. The increased level of Hcy, hs-CRP and NLR are independent risk factors of CAS.
