摘要
目的:基于数据挖掘技术研究中药治疗脑卒中后肢体痉挛(PSS)的用药规律。方法:检索中英文数据库中国知网、维普数据库、万方数据库、中国生物医学文献数据库、PubMed、EmBase、Cochrane Library、Web of Science自建库至2022年5月8日发表的中药汤剂内服治疗PSS的临床研究文献,应用数据挖掘技术对药物进行频次分析、性味归经及功效分析、关联规则分析、复杂网络分析、系统聚类分析。结果:共纳入文献195篇,包括中药166味,药性以温、寒为主,药味以甘、苦、辛居多,归经以肝、脾、心多见;药物功效主要为补益药、活血化瘀药、祛风湿药及平肝熄风药,频次排名前5位的中药为白芍、当归、川芎、地龙、黄芪;挖掘出15个核心药物组合,其中支持度最高的药对是当归-黄芪(补虚药-补虚药),置信度最高的药对是红花-桃仁(活血化瘀药-活血化瘀药);得出4类处方,为桂枝汤、天麻钩藤饮、大秦艽汤、补阳还五汤诸方所化裁。结论:中药治疗PSS以益气养血柔筋药为主,兼以活血化瘀、熄风止痉、祛风通络等药,标本兼顾、辨证施治。
Objective:To study the administration rule of Chinese medicine in treating post-stroke spasticity(PSS)based on data mining technology.Methods:The clinical research literatures on the treatment of PSS by internal administration of Chinese medicine decoction were retrieved from Chinese and English databases,including China National Knowledge Infrastructure(CNKI),VIP Database,Wanfang Database,Chinese Biomedical Literature Database(CBM),PubMed,EmBase,Cochrane Library and Web of Science,with a publishing date from the establishment day to May 8,2022.The data mining technology was used to analyze the frequency,nature,flavour,channel entry,curative effect,association rule,complex network and systematic cluster.Results:A total of 195 literatures were included,including 166 Chinese herbs.The medicinal nature of the herbs was mainly warm and cold,and the medicinal flavor was mostly sweet,bitter and pungent.The common channel entries were to the liver,spleen and heart,and the main curative effects were to supplement and boost,invigorate blood and dissolve stasis,remove wind-damp,and calm the liver and extinguish wind.The top five Chinese herbs in frequency were Paeoniae Radix Alba,Angelicae Sinensis Radix,Chuanxiong Rhizoma,Pheretima and Astragali Radix.Fifteen core herbal combinations were obtained,among which the herbal pair with the highest degree of support was Angelicae Sinensis Radix-Astragali Radix(deficiency supplementing herb-deficiency supplementing herb),and the herbal pair with the highest degree of confidence was Carthami FlosPersicae Semen(blood invigorating and stasis dissolving herb-blood invigorating and stasis dissolving herb).Four kinds of formulas were obtained,which were modified by Guizhi Decoction,Tianma Gouteng Decoction,Daqinjiao Decoction and Buyang Huanwu Decoction.Conclusion:In the treatment of PSS,Chinese medicine mainly emphasizes boosting qi,enriching yin and softening tendons,and also adopts the methods of invigorating blood and dissolving stasis,extinguishing wind and arresting convulsion,an
