血清胱抑素C评估IgA肾病患者病理损伤及早期肾损害的价值 被引量:3

The value of serum cystatin C in the evaluation of pathological injury and early renal damage in patients with IgA nephropathy

摘要 目的研究血清Cys C评估IgA肾病患者病理损伤与早期肾损害的价值。方法回顾性分析2019年10月-2021年10月于云南省第一人民医院肾内科经肾穿刺活检确诊为IgA肾病的患者200例。根据Lee氏分级标准及eGFR水平进行分组,收集患者相关临床及病理资料。结果在Lee氏分级分组中,随着病理损伤程度的加重,患者血清Cys C、SCr、BUN、UA水平逐渐升高,各组中血清Cys C的异常检出率均高于SCr、BUN、UA(P<0.001)。在肾功能分组中,随着eGFR的下降,患者血清Cys C、SCr、BUN、UA水平逐渐升高,肾功能正常组及轻度肾损害组患者血清Cys C的异常检出率高于SCr、BUN、UA(P<0.001)。Pearson相关性分析显示:血清Cys C与Lee氏分级呈正相关(r=0.480,P<0.001),与eGFR呈负相关(r=-0.870,P<0.001)。结论血清Cys C是评估IgA肾病患者病理损伤及早期肾损害的灵敏指标。 Objective To study the value of serum CysC in evaluating pathological injury and early renal damage in patients with IgA nephropathy.Methods A retrospective analysis was performed on 200 patients diagnosed with IgA nephropathy through renal biopsy in the Department of Nephrology,The First People s Hospital of Yunnan Province from October 2019 to October 2021.The patients were divided into groups according to Lee s classification standard and eGFR level,and the relevant clinical and pathological data were collected.Results In Lee s grading group,the levels of CysC,SCr,BUN and UA increased gradually with the aggravation of pathological injury,and the abnormal detection rate of serum CysC in each group was higher than that of SCr,BUN and UA(P<0.001).In the renal function group,the levels of CysC,SCr,BUN and UA increased gradually with the decrease of eGFR.The abnormal detection rate of serum CysC in normal renal function group and mild renal damage group was higher than that of SCr,BUN and UA(P<0.001).Pearson correlation analysis showed that there was a positive correlation between serum Cys C and Lee grade(r=0.480,P<0.001),and a negative correlation between serum Cys C and eGFR(r=-0.870,P<0.001).Conclusions Serum CysC is a sensitive index to evaluate the degree of pathological injury and early renal damage in patients with IgA nephropathy.
