摘要
目的探究利用二维斑点追踪技术得到的左室整体纵向应变(global longitudinal strain,GLS)参数能否预测冠心病患者冠脉存在严重狭窄。方法对入组的117例冠心病患者,行经胸超声心动图检查得到GLS参数。以冠状动脉造影(coronary arteriography,CAG)结果作为“金标准”,根据CAG结果将至少存在一支冠状动脉直径狭窄率≥70%者分为冠脉存在严重狭窄组和冠脉无严重狭窄组,比较组间GLS绝对值的差异。绘制受试者工作特性(ROC)曲线,评估曲线下面积(AUC),得到最佳截断值及对应的敏感度、特异度、约登指数及比值比(OR),讨论利用GLS预测冠心病患者存在冠脉严重狭窄(直径狭窄率≥70%)的可行性及临床价值。结果冠脉存在严重狭窄组平均GLS绝对值低于冠脉无严重狭窄组(P<0.001);ROC曲线分析AUC=0.757,敏感性为0.863,特异性为0.576,最佳截断值为-21.75%,约登指数为0.439,OR值为8.557。结论超声心动图二维斑点追踪技术得到的GLS参数对预测冠心病患者是否存在冠脉严重狭窄具有重要价值。
Objective To explore whether global longitudinal strain(GLS)parameters of left ventricle obtained by two-dimensional speckle tracking technology can predict severe coronary stenosis in patients with coronary heart disease.Methods The GLS parameters were determined by echocardiography of the 117 patients with coronary heart disease.Using the results of coronary arteriography(CAG)as the"gold standard",those with at least one coronary artery diameter stenosis rate≥70%were divided into severe coronary artery stenosis(SCAS)group and no severe coronary stenosis(NSCAS)group according to the results of CAG,and the difference of GLS value between the two groups was compared.The receiver operating characteristic(ROC)curve was plotted and the area under the curve(AUC)was evaluated to obtain the optimal cut-off value and the sensitivity,specificity,Youden's index and odds ratio(OR)value,to discuss the feasibility and clinical value,of using GLS to predict severe coronary artery stenosis(diameter stenosis rate≥70%)in patients with coronary heart disease.Results SCAS group's mean value of GLS was lower than NSCAS group(P<0.001).The AUC of ROC curve was 0.757,the sensitivity was 0.863,the specificity was 0.576,the cut-off value was-21.75%,Youden's index was 0.439,and the OR value was 8.557.Conclusion The GLS obtained by two-dimensional speckle tracking technique has great value in judging whether there is severe coronary stenosis in patients with coronary heart disease.
