摘要
目前研究认为高血压是脑小血管病(cerebral small-vessel disease,CSVD)发生及进展的独立危险因素,而血压变异性作为衡量血压变化的重要指标,与脑小血管病的发病机制如脑组织低灌注、血管内皮细胞损害及炎症反应等密切相关,其可以更好地反应靶器官的受损情况,且不同类型的血压变异性指标对脑小血管病总负荷评分影响存在差异。但是,血压变异性与CSVD总负荷的关系尚不清楚。本文通过对血压变异性与脑小血管病总负荷评分之间的研究进展进行综述,发现血压变异性尤其是24 h收缩压变异性、夜间收缩压变异性等与脑小血管病总负荷之间密切相关,并且通过控制血压波动可能会减轻脑小血管病总负荷且改善脑小血管病患者认知功能障碍,为早期识别、干预及改善脑小血管病患者的预后提供更多思路。
Current research suggests that high blood pressure is an independent risk factor of occurrence and progression of cerebral small-vessel disease.Blood pressure variability,one of the important indicators to measure blood pressure change,is closely associated with the pathogenesis of CSVD such as cerebral hypoperfusion,vascular endothelial cell damage and inflammation.Therefore,it can better indicate target organ damage.In addition,different types of BPV indicators have different effects on the total brain MRI load score in patients with CSVD.However,the relationship between BPV and total imaging load of CSVD remains unclear.This article reviews the research progress on the relationship between BPV and brain MRI total load score in patients with cerebral small vessel disease to provide more clues for early identification,intervention and improvement of prognosis of patients with CSVD.
