摘要
目的探討全髖關節置換術(THA)後脫位的原因及防治措施。方法回顧分析2006年6月~2012年12月3例THA術後脫位患者的術前病因、脫位原因、治療經過及術後預防再次脫位採取的措施。結果術後經6個月~6年隨訪,其中1例未再發生脫位。另2例在1個月内再次出現脫位,後經關節翻修術,未再出現脫位。結論髖關節周圍軟組織失衡、患肢主動或被動活動範圍過大、假體安裝位置不當是脫位的常見原因;脫位後患者可通過麻醉下行閉合復位治療,若假體位置不當者應考慮手術翻修;術前和術後對患者進行有關THA的康復知識教育是預防脫位的重要措施。
Objective To investigate the cause and prevention of dislocation after total hip arthroplasty(THA).Methods Retrospective study of 3 patient with dislocation after THA from June 2006 to December 2012;preoperative etiology of disease,cause of dislocation,treatment process and preventive measures undertaken for postoperation dislocation were randomly analyzed.Results During the 6 months to 6 years follow up,1 patient walked well without relapse of dislocation,in other 2 patients dislocation reoccurred in 3months,but got stable after revision surgery.Conclusion The major causes of dislocation after THR includes unbalance in peri-articular soft tissue,excessive active and passive movement of extremities and incorrect fitting of prosthesis.Majority of dislocations were corrected by closed reduction,revision surgery should be considered in case of severely misplaced prosthetic position.Pre and post operation rehabilitation knowledge education is key measures to prevent dislocation.
