龙州县HIV-1分子传播网络特征及影响因素分析 被引量:3

Characteristics and influencing factors of HIV-1 molecular transmission network in Longzhou County

摘要 目的 分析广西崇左市龙州县HIV/AIDS患者中HIV-1分子传播网络特征,并探讨网络形成的相关影响因素。方法 对2003-2021年龙州县报告的HIV/AIDS患者采集血样,通过DNA提取、pol区扩增、测序及序列整理,使用HIV-TRACE筛选最佳基因距离及构建分子传播网络。对分类资料进行χ^(2)检验,使用Logistic回归模型分析入网和高连接度个体的影响因素。结果 共收集到545份研究样本,成功获得361例HIV/AIDS患者的HIV-1 pol区序列。HIV亚型以CRF01_AE(88.09%)为主,其次为CRF07_BC(5.26%)和CRF08_BC(3.88%)。多因素结果显示,无性病史(OR=3.238,95%CI:1.134~10.642)、已婚/同居(OR=1.953,95%CI:1.03~3.758)的患者更容易入网;年龄≥50岁(OR=2.656,95%CI:1.394~5.109)的患者更容易成为该网络的高连接度个体,而高中/中专及以上(OR=0.251,95%CI:0.057~0.784)更不容易成为高连接度个体。结论 2003-2021年,龙州县艾滋病传播的关键人群为年龄≥50岁及低文化程度的患者,当地的相关部门应加强对该人群的治疗和管理及靶向干预,以减少HIV的二代传播。 Objective To analyze the characteristics of the HIV-1 molecular transmission network among people living with HIV/AIDS(PLWHA) in Longzhou County, Guangxi and explore the associated factors of transmission networks. Methods Blood samples were collected from PLWHA in Longzhou County from 2003 to 2021, through DNA extraction, HIV-1 pol amplification and sequencing. We used HIV-TRACE to screen the optimal gene distance of molecular transmission networks and to form transmission networks. A chi-square test and multiple logistic regression models were used to analyze the associated factors of transmission cluster and high-degree individuals. Results A total of 545 blood samples were collected and the HIV-1 pol sequences of 361 HIV/AIDS patients were obtained by amplification and sequencing. The main subtype of HIV was CRF01_AE(88.09%), followed by CRF07_BC(5.26%) and CRF08_BC(3.88%). In a multivariate analysis, patients who were without a history of sexually transmitted diseases(OR=3.238,95%CI: 1.134-10.642) and married or cohabiting(OR=1.953,95%CI: 1.03-3.758)were more likely to be members of transmission cluster. Patients aged 50 years or more(OR=2.656,95%CI: 1.394-5.109) were more likely to be high-degree individuals in the clusters. Patients who were with higher education(OR=0.251,95%CI: 0.057-0.784) were less likely to be high-degree individuals in the clusters. Conclusions From 2003 to 2021, PLWHA aged ≥50 years and with low education levels were the key population of HIV-1 transmission in Longzhou County. The local department should strengthen treatment and management interventions for the key population to reduce the secondary transmission of HIV.
