两种内固定治疗青壮年不稳定型股骨颈骨折的有限元分析 被引量:1

Finite element analysis of two internal fixations in the treatment of unstable femoral neck fractures in young adults

摘要 目的分析有限元模型下3枚空心钉固定和3枚空心钉联合1/3管型内侧支撑钢板固定治疗青壮年不稳定型股骨颈骨折的生物力学性能。方法对1名28岁志愿者的股骨CT数据进行三维重建,建立PauwelsⅢ型股骨颈骨折内固定的有限元模型,比较3枚空心钉和3枚空心钉联合1/3管型内侧支撑钢板的应力分布、应力峰值,以及股骨的位移分布、位移峰值。结果(1)2种内固定的应力均分布于股骨颈骨折断端周围,应力峰值:3枚空心钉模型为40.98 MPa,联合1/3管型内侧支撑钢板模型的空心钉、钢板及配套螺钉分别为28.92、22.58、51.13 MPa。(2)2种内固定的股骨最大位移均集中于股骨头顶端,股骨位移峰值:3枚空心钉模型为0.93 mm,联合1/3管型内侧支撑钢板模型为0.68 mm。结论与3枚空心钉固定相比,有限元模型下3枚空心钉联合1/3管型内侧支撑钢板治疗青壮年不稳定型股骨颈骨折的力学传导性更好,内固定及骨折端稳定性更高。 Objective To analysis the biomechanical properties of three cannulated screws fixation and three cannulated screws fixation combined 1/3 tubular medial support plate fixation in the treatment of unstable femoral neck fractures in young adults under finite element model.Methods The femoral CT data of a 28 year old volunteer was selected for three-dimensional reconstruction,the finite element model for internal fixation of Pauwels type-Ⅲfemoral neck fracture was established,the stress distribution,stress peak,and the displacement distribution,displacement peak of the femur were compared between three cannulated screws and three cannulated screws combined with 1/3 tubular medial support plate.Results(1)The stress of the two kinds of internal fixation were distributed around the fractured end of the femoral neck fracture.The peak stress:it was 40.98 MPa for the three cannulated screws model;in the three cannulated screws combined with 1/3 tubular medial support plate model,the cannulted screw,plate and the matching screw were 28.92,22.58,51.13 MPa,respectively.(2)The maximum displacement of the femur of the two internal fixations was concentrated on the top of the femoral head,and the displacement peaks of the femur were 0.93 mm with three cannulated screws fixation model,and 0.68 mm with three cannulated screws combined 1/3 tubular medial support plate fixation model,respectively.Conclusions Compared with three cannulated screws fixation,three cannulated screws combined with 1/3 tubular medial support plate fixation are used to treat the unstable femoral neck fractures of young adults under the finite element model,which have better mechanical conductivity and higher stability of internal fixation and fracture end.
