乳腺钙化在乳腺X射线摄影诊断中的应用准确性分析 被引量:1

摘要 目的:分析乳腺X射线摄影诊断中乳腺钙化特征提高诊断准确性的价值。方法:选取2020年1月—2022年4月济宁市兖州区人民医院收治的54例乳腺病变患者,均经病理诊断确诊为乳腺病变,对其实施影像学检查后发现,患者病变部位均存在不同程度的钙化特征,根据影像学结果中显示的钙化程度以及病变性质将其分为恶性钙化组(43例)和良性钙化组(11例),对所有患者进行乳腺X射线摄影诊断,以病理诊断结果为金标准,分析乳腺X射线摄影诊断的诊断效能,比较乳腺良性钙化病变和乳腺恶性钙化的病灶特征以及钙化情况。结果:54例患者经乳腺X射线摄影诊断后结果显示乳腺恶性钙化和良性钙化分别为44例和10例,乳腺X射线摄影诊断准确率、灵敏度和特异度分别为94.44%、97.67%和81.82%,与病理诊断结果高度一致性(Kappa=0.823);乳腺恶性钙化患者病灶直径≥1 cm、病灶形态不规则、病灶边缘比较模糊,检出率较乳腺良性钙化组高,差异显著(P<0.01);并且乳腺X射线摄影诊断结果显示乳腺恶性钙化组患者钙化病灶数量≥10个/cm^(2),钙化病灶呈现出了等密度钙化特征,节段性/线性分布的检出率显著高于乳腺良性钙化组(P<0.01)。结论:临床上对乳腺钙化患者实施诊断时应用乳腺X射线摄影诊断,能够准确鉴别乳腺恶性钙化病灶和良性钙化病灶,准确率较高,建议推广应用。 Objective To analyze the value of improving the diagnostic accuracy of breast calcification and breast calcification in mammography.Methods 54 patients with breast lesions admitted to Yanzhou District People's Hospital of Jining City from January 2020 to April 2022 were selected as the study subjects.All of them were diagnosed as breast lesions by pathological diagnosis.After imaging examination,it was found that there were different degrees of calcification characteristics in the lesions of the patients.According to the degree of calcification and the nature of the lesions,they were divided into malignant calcification group(43 cases)and benign calcification group(11 cases).Mammography was performed on all patients,and pathological diagnosis results were taken as the gold standard,to analyze the diagnostic efficiency of mammography and compare the features of benign and malignant breast calcification.Results 44 cases of malignant calcification and 10 cases of benign calcification were diagnosed by mammography.The accuracy,sensitivity and specificity of diagnostic mammography were 94.44%,97.67%and 81.82%,respectively.It was highly consistent with pathological diagnosis(Kappa=0.823).The lesion diameter of breast malignant calcification was≥1cm,the shape of the lesion was irregular,the edge of the lesion was relatively blurred,and the detection rate was higher than that of the benign calcification group,and the difference was significant(P<0.01).Moreover,mammography diagnosis results showed that the number of calcified lesions in the malignant calcification group was not less than 10/cm^(2),the calcified lesions presented the characteristics of isodensity calcification,and detection rate of segmental/linear distribution was significantly higher than that of the benign calcification group(P<0.01).Conclusion The mammography in clinical diagnosis of breast calcification patients can accurately identify breast malignant calcification lesions and benign calcification lesions.The accuracy rate is high.It is recommended
