Effect of Simultaneous Integrated Boost and Sequential Intensive-modulated Radiotherapy on Locally Advanced Esophageal Cancer and Its Impact on 3-year Survival Rate

摘要 目的探讨全程局部同步推量调强放疗(SIB-IMRT)治疗局部晚期食管癌的效果及对3年生存率的影响。方法回顾性分析2018年8月—2019年6月收治的局部晚期食管癌108例的临床资料,根据治疗方法分为研究组和对照组各54例,研究组采用SIB-IMRT治疗,对照组采用序贯加量调强放疗(IMRT)治疗,比较2组临床疗效、糖类抗原125(CA125)、癌胚抗原(CEA)、细胞角化素蛋白片段19(CYFRA21-1)、免疫功能[CD3+、CD4+、CD8+和CD4+/CD8+]、3年生存率。结果2组近期客观缓解率比较差异无统计学意义(P>0.05)。治疗后研究组CA125、CEA、CYFRA21-1和CD8+水平低于对照组、CD3+、CD4+、CD4+/CD8+高于对照组(P<0.05,P<0.01)。研究组3年生存率高于对照组,放射性食管炎和放射性肺炎发生率低于对照组(P<0.05)。结论SIB-IMRT治疗局部晚期食管癌可显著降低血清肿瘤标志物水平和放射性肺炎发生率,减少对免疫功能的影响,提高远期生存率。 Objective To compare the effect of simultaneous integrated boost(SIB)and sequential intensity-modulated radiotherapy(IMRT)on locally advanced esophageal cancer and its impact on 3-year survival rate.Methods The clinical data of 108 patients with locally advanced esophageal cancer treated between August 2018 and June 2019 were retrospectively analyzed.According to the treatment methods,the patients were divided into the research group(n=54)and the control group(n=54).The research group was treated with SIB-IMRT,while the control group was treated with sequential IMRT.The clinical curative effect,carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA),cytokeratin fragment 19(CYFRA21-1),immune function indicators[CD3+,CD4+,CD8+,CD4+/CD8+]and 3-year survival rate were compared between the two groups.Results There was no significant difference in short-term objective remission rate between the two groups(P>0.05).After treatment,the levels of CA125,CEA,CYFRA21-1 and CD8+in the research group were lower than those in the control group(P<0.05),while the levels of CD3+,CD4+,and CD4+/CD8+in the research group were higher than those in the control group(P<0.05,P<0.01).The 3-year survival rate of the research group was higher than that of the control group,and the incidence of radiation esophagitis and radiation pneumonia was lower than that of the control group(P<0.05).Conclusion SIB-IMRT in the treatment of locally advanced esophageal cancer can significantly reduce the level of serum tumor markers and the incidence of radiation pneumonia,reduce the impact on immune function,and improve the long-term survival rate.
