摘要
目的 介绍并探讨阴茎根部蝶形固定在儿童隐匿性阴茎成形术中的应用效果。方法回顾性分析2020年6月至2020年8月在我科行阴茎根部蝶形固定隐匿性阴茎成形术(“蝶形固定法”)治疗的80例患儿的临床资料,分析其手术时间、手术前后阴茎长度变化、并发症发生情况、术后阴茎回缩率及效果满意度。结果 80例隐匿性阴茎患儿均顺利实施蝶形固定法,平均手术时间(61.4±8.8)min。术后阴茎长度平均(5.61±1.15)cm,较术前(2.14±0.53)cm明显改善,差异有统计学意义(P<0.001)。术后1个月复诊,阴茎水肿9例,伤口裂开未愈合1例,未见伤口感染及血肿形成;术后半年随访见阴茎瘢痕赘生5例,阴茎回缩1例,外观满意度为91.25%。结论 阴茎根部蝶形固定在儿童隐匿性阴茎成形术中应用效果较好,术后外观满意度高,再回缩率低,可作为进一步研究和推广的方法之一。
Objective To introduce and discuss the effect of butterfly fixation at the root of penis in children’s concealed phalloplasty. Methods The clinical data of 80 children with concealed penis underwent concealed phalloplasty with butterfly fixation of penis root( "butterfly fixation") in our hospital from June 2020 to August 2020 were analyzed retrospectively. The operation time, penis length changes, complication occurrence, postoperative penile retraction rate and effect satisfaction were observed. Results All 80 children with concealed penis were accomplished the concealed phalloplasty with butterfly fixation successfully. The average operation time was(61.4±8.8) min. The length of the penis was(5.61±1.15) cm after the operation, which was improved obviously than(2.14±0.53) cm before the operation, and the difference was statistically(P<0.05). Penile edema was found in 9 cases and wound dehiscence and nonunion in 1 case, no wound infection and hematoma formation occurred in one month after the operation. There were penile scars in 5 cases and penile retraction in 1 case, and the appearance satisfaction was 91.25% in 6 months follow-up after the operation. Conclusion Butterfly fixation of penis root is effective in concealed phalloplasty of children with high postoperative appearance satisfaction and low retraction rate, which can be used as one of the methods for further research and promotion.
